Team Leader (CRHTT)– Lead, Inspire, Transform

We are looking for an experienced and passionate Team Leader to take the helm of a high-performing Crisis team based in Worthing, driving excellence in patient care and service delivery. If you're a confident decision-maker, a mentor, and a problem-solver who thrives in a fast-paced healthcare environment, this is the perfect opportunity for you!

Working alongside the Clinical Nurse Specialist and under the leadership of the Service Manager, the Team Leader will play a pivotal role in ensuring the Crisis Team delivers holistic, patient-centred, and safe care across the Urgent Care Pathway. You will work closely with the Mental Health Liaison Team at Worthing General Hospital and The Haven at Meadowfield Hospital to provide seamless support to those in crisis.

With an exciting move to the new Integrated Care Centre in central Worthing on the horizon, this role will be at the forefront of shaping how community neighbourhood teams collaborate. The successful candidate will be instrumental in streamlining pathways between services, enhancing patient experience, and improving overall care delivery.

 

Main duties of the job

Knowledge/Experience

  • Significant experience of and demonstrable achievement in clinical practice
  • Experience of providing professional support/supervision and motivation of staff
  • Understanding of Health and Social Care Governance
  • Experience of managing staff
