Job overview

Are you currently a band 5 looking for development opportunities or a band 6 with a specialist interest in integration, hospital discharge and flow. This is an exciting time to join the integrated discharge team, we have a range of transformational projects we are developing that you could be part of.

Main duties of the job

  • To assess, plan and coordinate safe, timely discharges via the most appropriate discharge pathway to meet the patients' required needs.
  • Manage a caseload of patients with varying diagnosis and or /complex needs with a range of disabilities including physical, learning and mental health disabilities.
  • To educate and train the team on the role of OT and provide day to day management for junior staff, ensuring supervision and annual review.
  • To participate in the planning, development and evaluation of the discharge team service within the acute trust and the wider integrated services.
  • Take a lead for information management to ensure high quality data which is recorded, evaluated & reported in a timely manner.

Detailed job description and main responsibilities

To plan and implement appropriate discharges through comprehensive holistic  assessments and knowledge of discharge pathways, working 5 days across 7 working days
