This unique position offers you the opportunity to work as part of a highly skilled community workforce who are the cornerstone of safe patient care. You will work collaboratively with the multi-disciplinary team to visit patients in their own homes and care homes to avoid unnecessary hospital admissions.

We have lots of training and development pathways at every level through from band 3 to band 8. We provide a preceptorship program to support you in your role and ongoing training and development. 


We use electronic clinical records through EMIS and a work IPad and IPhone will be provided.

 

 

 

 

 

Main duties of the job

Job Purpose:
• The post holder will visit adult patients in their own home who have an identified nursing need including those with a long-term condition, complex care needs and needing care at home and the end of life.
• The post holder will use an electronic patient held record system, EMIS as a record of care and all our nurses are provided with a work Iphone and Ipad for mobile working.
• The community nurse will use a range of nursing skills including venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control and on successfully attaining all the competencies required will be reviewed for a Band 6 Senior Community Nurse post.

Detailed job description and main responsibilities

We are seeking a highly motivated and ambitious Registered Nurse to join the wider Community Nursing Team which includes Community Matrons, District Nurses, Registered Nurses, Nursing Associates and Healthcare Assistants. You will work under the direction of a District Nurse to assess, implement and evaluate person centred care for a caseload of patients. You will undertake clinical skills including venepuncture, wound care, catheterisation and end of life including syringe drivers. 

Post holders will be available to work Monday to Sunday between 8am and 8pm. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Person specification

Education / Training / Qualifications

Essential criteria
  • Registered Level 1 Nurse with current Nursing and Midwifery Council registration
  • Educated to diploma or level 6 / degree level in relevant area
  • Supervisor / Mentorship qualification (within the first 2 years of post)
  • Evidence of continuing professional development
  • Area of special interest in the management and treatment of long-term conditions and willing to undertake additional education in this area

Knowledge and Experience

Essential criteria
  • Ability to assess nursing needs and implement nursing care and treatment
  • Ability to prioritise visits in accordance with patient clinical need
  • Knowledge of recent developments within health and social care
