Job overview

About the Role: As a vital member of our multidisciplinary team, you will play a crucial role in delivering top-notch patient care. Your efforts will focus on implementing evidence-based improvements to identify frailty early and mitigate associated risks.
Key Responsibilities:
- Proactive Case Finding: Identify patients living with frailty within urgent and emergency care settings.
- Comprehensive Geriatric Assessment: Develop thorough assessments to understand each patient's unique needs.
- Discharge Planning: Initiate and manage discharge plans, including social care packages, fast track, end-of-life care, and rapid discharges, all aimed at avoiding unnecessary hospital stays

Knowledge
Essential criteria
- Excellent communication and time management skills
- Ability to change, manage and motivate others
- Knowledge of audit and research
- Specialist knowledge of frailty developed through significant experience or a diploma in frailty or has a willingness to work towards.
- Knowledge in assessing the needs of patients with complex health and social care needs
Desirable criteria
- Understanding of frailty in same day urgent care.
Qualifications
Essential criteria
- First level registered nurse.
- First degree
- Teaching qualification
Desirable criteria
- Masters degree
- Community nursing experience
- Experience in HCOP
Further Training
Essential criteria
- Mandatory update
- Postgraduate diploma or willingness to work towards
- Ability to travel between all sites
- Computer skills.
Desirable criteria
- ECDL
Experience
Essential criteria
- Significant experience as a registered nurse
- Knowledge and experience of intermediate care services.
- Evidence of multidisciplinary working
Desirable criteria
- Knowledge of the legislation for discharge processes
- In depth knowledge of continuing health and social care policies
- Policy writing and interpretations.
- Change management skills.
- Recent participation in clinical audit and / or research project.
- Understanding of frailty and frailty syndromes
