Job overview

*PREVIOUS APPLICANTS NEED NOT APPLY*

An exciting opportunity has arisen to join the Wirral University Teaching Hospitals General Surgery team as we look to appoint a Patient Navigator or focussing on the Colorectal Service with a specific focus on the FIT pathway.

This role is vital in providing accurate data and assurance to the Trust by improving patient demographic data quality to minimise the clinical risk to patients.

Main duties of the job

  • This role would be focused on supporting the clinical team in real time tracking/chasing of patient progress through each stage of their pathway to ensure smooth and timely transition.
  • Other administrative support would also be supported for the clinical team within this role, including elements such as introducing standardised coded letters for patients who have been excluded from having cancer and improved data capture of the Somerset Cancer Register.
  • Working as part of a clinical team to ensure that patients are admitted efficiently for Endoscopy appointments, investigative treatments, diagnostic procedures and inpatient and day case operations within national guidelines and in accordance with the Trust’s Patients Access, Booking and Choice Policy. This includes:
NHS Constitution, waiting time standards; Department of Health admission guidelines and booking rules.
  • To have overall responsibility for ensuring that patients are scheduled in accordance with principles and target waiting times as set out in the NHS Constitution and Health and Social Care Bill 2011 (e.g. 18 Week Referral to Treatment, Going Further on Cancer Waits etc.), expediting patient pathways where necessary.
