Job overview

An exciting opportunity has arisen within the Integrated Safeguarding Team  support the provision of high quality, safe and effective patient care and continuous improvement of the patient experience by providing effective, professional nurse leadership across the Trust as the Matron for Dementia Care.

The post holder will promote evidence-based dementia care across the organisation thereby improving patient, carer, and staff experience. 

 The post holder will play a key role in the implementation of key initiatives related to dementia care, with focus for the development and sustaining of a culture of continued learning and improvement amongst all staff groups, but with particular focus upon nursing, and being visible across wards and departments and championing dementia care.

 The post holder will take a lead role for instigating, facilitating, implementing, and monitoring change management initiatives as directed from the Head of Safeguarding.

 The post holder will lead on key work streams, in partnership with other health and care professionals, patients and their families to support the development of a dementia friendly environment, the drive to improve staff knowledge and skills, and the delivery of key quality and performance indicators in support of the Trust dementia agenda.

 They will provide strong professional leadership, enabling and empowering Trust staff to develop practice in the development of high quality patient centred services.

Main duties of the job

To lead and facilitate the delivery of the work streams as part of the Trust’s Dementia Strategy.

Work collaboratively with partners agencies to support the Trusts Dementia agenda

To provide a specialist resource of advice and expertise to patients, their families, carers, and staff as appropriate.

Take the lead on projects / initiatives to improve and enhance the patient / carer experience as identified through captured intelligence, ensuring that change management principles are recognised and implemented.

To develop relationships with others across both professional and primary care boundaries, e.g., local / national agencies, social care, key stakeholders, and partners. 

To work closely with Trust Safeguarding teams and MDT’s to consider when appropriate and implement Mental Capacity Assessments and Best Interest Decision Making when specific decisions need to be made with regard to a patient’s clinical care.

Provide reports for forums, committees and Trust Board as and when required.

Represent the Trust at meetings across the health and social care economy where dementia is a key agenda item

To inform decisions and make judgments on recommendations relating to future actions by translation of national and regional policy initiatives into working practices 

Manage own workload with the ability to work to deadlines, prioritising and initiating own workload.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

 

If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Person specification

Education & Qualifications

Essential criteria
  • Registered practitioner
  • Current NMC registration RN or RMN with demonstrable experience within a dementia care environment
  • Masters Level study or equivalent transferable experience
  • Mentorship course or equivalent qualification
  • Evidence of recent continuing personal learning/professional development
Desirable criteria
  • Masters Level study in area of dementia and /or frailty
  • Management or leadership qualification

Knowledge

Essential criteria
  • Knowledge of current NHS policies/processes and structures relating to Dementia and Frailty
  • Knowledge of Dementia agendas both NHS and voluntary led sector
  • Good knowledge of functional mental health issues and psychology of ageing and frailty
  • Good understanding of dementia, and BPSD and therapeutic interventions
  • Good understanding of Safeguarding/MCA/DOLS

Experience

Essential criteria
  • Development of care pathways, standards and service models
  • Expert knowledge of MCA and DOLs application
  • Experience of working across statutory bodies and organisations
  • Experience in dealing with highly emotive and confidential information and advising on management of complex cases
  • Experience of working with Training and Development, delivering training and monitoring effectiveness
  • Ability to influence, negotiate and provide challenge to internal and external partners in a constructive and appropriate manner where required
Desirable criteria
  • Experience in undertaking research and audit

Skills and Abilities

Essential criteria
  • Ability to impart highly complex and sensitive information to others with different levels of understanding
  • Ability to write reports for organisational assurance and escalate concerns effectively
  • Demonstrates excellent oral and written communication skills including presentations and publications
  • Able to communicate effectively with people who use Trust services, carers, families and members of the public
  • Well developed skills in audit evaluation of service provision and outcomes for people who use Trust services
  • Ability to write, revise and implement policies, procedures, standards and guidelines
  • Experience of leading service planning, development and strategy
  • Proven leadership skills and senior management experience

Work Related Circumstances

  • Moderate level of physical effort. Post will require walking across large hospital sites, attending on site meetings at a variety of locations and driving to multi-agency meetings off site
