Prif ddyletswyddau'r swydd

  1. Assess, plan, implement, and evaluate care for alcohol dependent patients with complex needs, making necessary adjustments.
  2. Identify patients at high risk from their alcohol use using the AUDIT assessment and take appropriate action in collaboration with the interdisciplinary team.
  3. Deliver evidence-based interventions, including motivational interviewing and withdrawal management, to support patients in reducing alcohol-related harm.
  4. Educate patients and families about healthy lifestyles and managing risks to health and well-being.
  5. Ensure accurate, up-to-date documentation using electronic health records and maintain compliance with the NMC Code of Professional Conduct.
  6. Communicate effectively with patients, staff, and external organizations, using strategies to overcome barriers and promote continuity of care.
  7. Support research, audits, and quality initiatives to enhance patient care and nursing practice.
  8. Oversee the delivery of a 7-day alcohol care service model to ensure consistent and equitable coverage for patients across all settings.

Although this position is a 29 months fixed-term contract, the actual contract duration will be adjusted to account for the time taken to advertise the post, complete the hiring process, and finalise pre-employment checks. This means the contract will begin when the candidate starts the role and may be slightly shorter than 29 months.

  • Relevant addiction experience /experience in delivering psychosocial interventions with patients
  • Non-medical prescribing qualification (e.g. V300)
