An exciting opportunity has arisen within the Paediatric Diabetes Team for a highly motivated, enthusiastic Clinical Support Worker to make a difference to the lives of children, young people and their families with diabetes at Hillingdon Hospital NHS Foundation Trust.
Working directly with families, children and young people both individually and within groups as well as working within an established and extremely driven paediatric diabetes team to deliver a high standard of care to children and young people living with diabetes.

Main duties of the job
Main duties, tasks & skills required
To provide a high quality, safe, equitable and efficient service to children under the care of the Hillingdon Paediatric Diabetes team. Diabetes training will be provided in post.
The role involves assisting qualified members of the team, to provide interventional support to children, young people and their families in hospital, their homes, at school (main stream and specialist school settings), and other community environments. All of these children are living with diabetes and may also have other possible comorbid conditions such as autism, and or learning difficulties, coeliac disease, thyroid abnormalities.
The role will involve independent and team working, clinical competency too, for example, carry out school visits and home visits to support children/young people and their families in a variety of aspects of their diabetes management. For example, supporting families with diabetes technologies, accessing disability living allowance or charity funding where appropriate, and organising peer support events. The role would include delivering elements of planned teaching programs and liaising with the multi-disciplinary team including contributing, where appropriate, to interactive health care plans and safeguarding discussions. The role will also involve clerical responsibilities including audits, updating patient notes and completing funding applications and orders for diabetes equipment.
