You will use your skills and knowledge to enhance current work on admission avoidance at the front door and facilitate early, supported discharge from our HSEP wards. We are looking for someone who has innovative ideas and is passionate about being an advocate and champion for preventing deconditioning, particularly for the cohort of patients with complex physical and mental needs.

The successful candidate is required to demonstrate the Trust values, the responsibilities of the job description and person specification.

In particular, you should have proven experience of leading teams including supervision and line management of junior colleagues, an enthusiasm for service development and be motivated to find new and innovative ways of working to meet the ever-changing demands of the service.
