Job overview

  • The post holder will work as a member of the NHS Lincolnshire Integrated Care Board Integrated Medicines and Optimisation Team, which is part of the Medical Directorate.

 

This role is responsible for providing the lead on duties as discharged by the Chief Pharmacist to develop, implement, and monitor standards for medication safety, safe use of medical devices and safe prescribing. This includes key activities to ensure the ICB meets Controlled Drugs (CD) responsibilities, managing relevant incidents involving medicines and prescribable products, including sharing knowledge and awareness of the patient safety agenda and learning from medicines and prescribing incidents and working with the area prescribing committee and Provider Trusts to assure Medicines Optimisation strategies provide safe, high-quality services for our patients. This also includes working with the Area Prescribing Committee and Provider Trusts to assure Medicines Optimisation strategies provide safe, high-quality services for our patients and promote a culture of medicines and medical device safety across the local health economy.

This role is for a qualified and registered pharmacy professional.

Main duties of the job

·    Lead on duties as discharged by the ICB Chief Pharmacist to develop, implement and monitor standards for medication safety, safe use of medical devices and safe prescribing.

·       Lead on activities to ensure the ICB meets Controlled Drugs (CD) responsibilities.

·   Manage relevant incidents involving medicines, prescribable products and medical devices.

 

·       Promote, influence and enhance a culture of medicines and medical device safety across the health economy by involving stakeholders, including patients and the public, to avoid medicines related harm.

·     Co-ordinate with other teams the development of medication safety awareness and learning across the ICB and within GP practices and providers.

 

·   Provide expert advice and regular updates to the area prescribing committee to promote safe and effective use of medicines

·     Provide support to lead APC pharmacist to develop and implement medicines safety strategy priorities, guidelines & policies.

 

  • Provide specialist support to the team regarding any medication incidents involving care homes or agencies. 

 

·     Interface with community and hospital pharmacy colleagues to ensure the patient journey with medicines is as efficient and uninterrupted as possible. Work with colleagues across the system to improve the safety of the discharge process and movement between different care sectors.

·   Work with the AMS System Lead and SRO to develop and implement Antimicrobial Stewardship across Lincolnshire in order to improve the appropriate prescribing of antimicrobials.

Essential & desirable

Essential criteria
  • MPharm degree (or equivalent experience), and educated to a post graduate level or equivalent level of experience of working at a senior level in specialist area
  • Diploma or MSc in clinical pharmacy or pharmacy practice, or equivalent level of experience of working at a senior level in specialist area
  • Current professional registration with the General Pharmaceutical Council
  • Evidence of post qualifying and continuing professional Development in a specialist area
  • Experience of working in more than one sector of pharmacy i.e. secondary care or community pharmacy or the private sector including primary care
  • Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement
  • Experience of managing change in clinical practice
  • Experience of influencing others, negotiating change, and motivating others within a team to improve patient care
  • Advanced therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
  • Able to plan, manage, monitor, advise and review therapeutic optimisation issues in core long term condition areas for individual patients
  • Knowledge and ability to produce, analyse, present and interpret in-depth analysis of highly complex prescribing and financial trends and local and national comparators.
  • Must be able to provide and receive highly complex, sensitive or contentious information, negotiate with senior stakeholders on difficult and controversial issues, and present complex and sensitive information to large and influential groups
  • Strategic thinking – ability to anticipate and resolve problems before they arise
  • Demonstrated capability to plan over short, medium and long-term timeframes and adjust plans and resource requirements accordingly
  • Must be able to prioritise own work effectively and be able to direct activities of others Experience of managing and motivating a team and reviewing performance of the individuals.
  • Experience of identifying and interpreting National policy. Experience of researching best practice (globally, private and public sector), interpreting its relevance and processes/ practices which could be implemented successfully to achieve system reform (advising on policy implementation).
  • Needs to have a thorough understanding of and commitment to equality of opportunity and good working relationships both in terms of day-to-day working practices, but also in relation to management systems.
  • Ability to travel across multiple sites where required
