Band 4 - 12 months fixed term
This is an exciting opportunity for a dedicated and compassionate person to join our Cardiac and Pulmonary Rehabilitation team to support the delivery of high-quality rehabilitation services to patients recovering from cardiac events and those with chronic pulmonary conditions. This role involves assisting physiotherapists in implementing individualized exercise programs, monitoring patient progress, and providing education on lifestyle modifications.
You will ideally have a knowledge of exercise delivery and experience working with people with long term conditions in a healthcare environment or rehabilitation setting.
Main duties of the job
To assist in the rehabilitation of patients within the community rehabilitation pathway by implementing individual treatment programmes and standard operating procedures as prescribed by a clinician. You will be required to work with indirect supervision, reporting back to patient progress to the team and highlighting any problems. You will assist in the case management, general administration and equipment management within the therapy service.

Essential criteria
- Experience delivering healthcare advice including exercise and lifestyle support
- Worked in the NHS or equivalent healthcare/rehabilitation setting at at least a Band 3 level.
- Able to check observations including blood pressure, heart rate, and oxygens saturations
- Knowledge of and ability to coach use of basic gym equipment safety.
Desirable criteria
- Worked within a rehabilitation service.
- Able to use clinical systems such as EMIS and Lorenzo
Knowledge
Essential criteria
- Can demonstrate experience working in a MDT within healthcare setting
- Experience of delivering rehabilitation with or without support from registered therapists
- Can demonstrate awareness of managing unwell patients and long term conditions
Desirable criteria
- Has experience of exercise rehabilitation for people with cardiac and pulmonary health conditions

Essential criteria
- Able to demonstrate empathy and ability to communicate with people effectively including those with communication barriers.
- Able to demonstrate excellent written and verbal skills.
