Main duties of the job

The 8a Lead for Patient Risk Stratified Follow Up Pathways (PSFU) and delivery of Personalised Care in cancer services is an innovative and dynamic role using clinical knowledge and expertise and experience in the use of digital tools.

Detailed job description and main responsibilities

•    Develop and embed personalised care interventions and Personalised Stratified Follow Up (PSFU) into cancer pathways by facilitating the collaboration between clinical and digital teams at UHS to maximise the benefits of digital tracking. The role will support the monitoring and sustained delivery of PSFU by measuring and evaluating the benefits. 

•    Facilitate cancer services by coordinating and supporting the governance and roll out of the remote surveillance by working across professional, departmental and Trust boundaries as required to develop and support patient-centred seamless care, with particular focus on the delivery of Personalised Care, patient self-management and wellbeing services.

This role will allow for flexible working with allowance for some home working. 
The candidate should have experience of cancer nursing, transformation programmes and the implementation of digital platforms in cancer services.
