Job overview
This is a key role within our Hub service, located at Fair Havens, overlooking beautiful Priory park.
This role coordinates and manages the activities provided by the Hub service, all whilst providing a wide range of administrative support to our staff and volunteers.
Please note that this role may close early if sufficient applications have been received.
Main duties of the job
You will work as part of the wider team of coordinators to ensure that services are streamlined with robust internal referral pathways.
This also involves liaising with the hospice multidisciplinary team to ensure that assessments are coordinated and monitored and that referrals to the service are appropriate.

Detailed job description and main responsibilities
- Assist with preparation of patient's assessment notes, referrals etc.
- Support Hub staff with patient questionnaires – completion, collection and inputting of questionnaires.
- Supporting the team by providing some judgement of situations which may require analysis. Including low-level emotional support when required on the phone and in person.
- Providing and receiving routine information requires tact skills where there are barriers to understanding due to cultural, mental health and physical support needs.
- Communicating sensitive topics with patients and families who may occasionally be emotional or experiencing distressing situations.
- Contact referrers and patients on the waiting list to acknowledge their referral and advise on the next steps.
- To monitor and respond to the website, email and phone enquires in a sensitive and professional manner.
- Understanding of a range of routine work procedures including standard operating procedures across both Wellbeing and Hub services.
- Undertakes surveys or audits, as necessary to own work.
- To provide administrative support at the weekly multidisciplinary team meeting including minutes and caseload management.
- Check SystmOne for patient referrals, and tasks daily.
- Ensure tasks are forwarded to the task recipients.

Experience
Essential criteria
- Demonstrable experience in a busy and varied administrative role
- Excellent Microsoft Office and IT skills, including Excel, PowerPoint, Word, Outlook
Desirable criteria
- Experienced SystmOne user
Qualifications
Essential criteria
- GCSE or equivalent in Maths & English, or equivalent
- Excellent written and oral communication skills.
- Excellent customer service skills
Desirable criteria
- Full UK Driving licence and access to own vehicle
