Health Care Support Worker

Band 3

Trosolwg o'r swydd

Bayman ward is a 25 bedded ward providing a service supporting stroke rehabilitation. The ward is nurse led, supported by Medical Practitioners and provides a progressive service which promotes multidisciplinary working. The philosophy is one that encourages staff participation at all levels and identifies opportunities for service and career development.

The successful applicant will be required to integrate flexibly across the ward with a particular focus on communication with the MDT to support a team approach to care. A key requirement of the role  will be to deliver safe high quality care that meets the needs of  increasingly complex patients in the community wards.

This will involve  joint working between all of our inpatient units and supporting community services to manage patient pathways safely and effectively and ensure that robust discharge plans are in place that support patients to live their lives as independantly as possible on discharge. There are close working relationships with medical practitioners and MDT which encourage a seamless and proactive approach to planning successful discharge.
