Are you a Band 5 Nurse who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced Band 5 nurse to support the changing face of community health care within our Rapid Response Team. As part of the Urgent Community Response and Virtual Ward initiative, your role will be essential in providing efficient, integrated, urgent care to our patients with care and respect.

These posts will be to cover either North & Mid Bedfordshire or South & Mid Bedforshire meaning there will be the expectation the applicant can flex to which area had the most demand and need. 

 

Through face to face patient visits you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions.

You will be working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community.

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. Consideration will also be given to staff who are newly qualified and are passionate about a career in community nursing. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.

The Trust aims to recruit and retain the best possible staff, who will make a positive contribution to the service, in return we offer excellent development opportunities and training that can be directed towards your needs/goals. You will have regular supervision, mentorship and support from within the Rapid Response Team and the Trust.

You must have a driving license and have access to a vehicle in order to carry out your duties.  You will be required to work a shift pattern covering 7 days a week from 08:00-20:00.

  • Demonstrate evidence-based practice and up-to-date clinical skills

  • Relevant teaching/mentorship qualification or commitment to undertake
  • Evidence of post-basic professional development

Experience

  • Understanding of the interface between primary and secondary care
  • Comprehensive understanding of the roles of others in health and social care

Knowledge/Skills

  • Literate in IT/Computer Skills
  • Evidence of knowledge of clinical governance
  • Knowledge of the principles of Safeguarding of Vulnerable Adults and Children
  • Knowledge of the principles of health promotion and prevention of illness
  • Good oral communication skills based on fluency in English language
