The Lifetime Service is a Community Nursing and Psychology Service for Children and young people life limiting, life threatening conditions and those who meet Children’s Continuing Care criteria.

The service is based in Kingswood, South Gloucestershire and currently provides a service across four Integrated care board (ICB) areas, supporting children aged 0-19 and their families in Bristol, South Gloucestershire, North Somerset, B&NES.

In addition, the service has a Continuing Care Team (CCT), which supports children at home with complex health needs and children who are technology dependent.

 

 

 

 

Main duties of the job

The purpose of this role is to undertake clinical shifts  working alongside the Lifetime support workers in the child’s home or school setting, observing their clinical competencies and undertaking annual clinical assessments to provide outstanding care and support to the children and their families who meet Continuing Care Criteria (Bristol, South Gloucestershire, North Somerset and B&NES). This role includes clinical support to the rota co-ordination team.

This role will take on the line management of a number of Lifetime Support Workers which includes management and assessment of the wider team, having regular one to one meetings, performance development reviews and absence management meetings to support the team leaders. Additionally, the successful candidate will assist in the delivery of clinical skills training days, upskilling support workers and ensuring the team are providing evidence based interventions inline with national guidance. 

The role will require travel across Bristol, North Somerset and South Gloucestershire, visiting service users within their own homes and occasionally in schools, in addition to meeting and liaising with other teams and services involved in the child's care.

Qualifications and training

Essential criteria
  • RSCN/RN Child registered on part 8 or 15 NMC register
  • Full UK Driving Licence & Access to a vehicle for work purposes
Desirable criteria
  • BSc (Hons) Community Children’s Nurse/relevant community experience i.e. Specialist Practitioner CCN or SCPHN
  • Facilitating Learning and Assessment in Practice Course or Equivalent

Skills, Experience and Knowledge Required

Essential criteria
  • Knowledge of professional agenda and NMC standards
  • Understanding of systems and processes in relation to safe service delivery
  • Experience of working as part of a multi-disciplinary team
  • Ability to work in a changing environment
  • Experience working as a Childrens Nurse
  • Evidence of continuing professional development (CPD)in areas relevant to Community
Desirable criteria
  • Children’s Continuing Care and/or community experience
  • Experience of complex rota’s to meet service and staffing demands.
