Consultant Psychiatrist in Psychotherapy

CNWL are currently looking for an experienced Consultant Psychiatrist to provide medical leadership in developing and managing the Westminster Psychotherapy service. This post is an established part time (6 sessions) replacement post due to the previous post holder leaving the Trust, and is based at both our Westminster CMHT HUB's at 7a Woodfield Road and 190 Vauxhall Bridge Road,
The post-holder will work within and alongside the Westminster community mental health teams to inform overall treatments, and to provide specialist psychological therapies assessments and treatment as part of integrated care pathways. The post-holder will also be working with a CEN Clinical Lead to provide specialist psychotherapy interventions.

Central and North West London NHS Foundation Trust (CNWL) specialises in caring for people with a wide range of physical and mental health needs.
Over recent years our catchment area has grown significantly, making the Trust’s population more diverse than ever. The area spans the communities of London, Milton Keynes and wider geographical areas of Buckinghamshire, Surrey, Kent and Hampshire. There are areas of great affluence as well as deprivation, and there are over 100 first-languages spoken in these communities.
Westminster is an inner London area with a mixed, diverse population with some of the most wealthy and most deprived areas of London. There is a high mental health morbidity and the local CCG is committed to a high spend on mental health services.
Westminster adult mental health service provides a range of mental health services for adults across the age range within the community and in hospital settings. Many services across the Boroughs of Westminster and Royal Borough of Kensington and Chelsea (K&C) are now integrated because of the synergies of the two patches, with members of the senior management often having overarching responsibilities.

CORE CLINICAL AND LEADERSHIP:
1. Managing psychotherapy referrals, as well as psychotherapy waiting lists in the Psychotherapy Service.
2. Providing specialist psychological therapy assessments and extended consultations. The post-holder will provide on a weekly basis one new psychotherapy assessment directly, two follow-up assessment consultations and one consultation for ongoing complex psychotherapy cases.
3. Holding a clinical caseload including –
• patients undergoing psychotherapy assessment;
• complex psychotherapy patients;
• patients in ongoing psychotherapy treatments including as provided by junior medical staff and any non-medical psychotherapists under the supervision of the post-holder (on average forty patients approximately).
4. Providing clinical and psychotherapy supervision for –
• 2 WTE Psychotherapy junior doctor posts (six-months psychotherapy placements usually at CT3 level) and,
• 1 WTE Psychotherapy Higher trainee (ST4-6) post
allocated to this Consultant post.
5. The post holder will also be providing regular supervision of psychotherapy cases of junior doctors (from other teams) during their rotation placement in Westminster services.
Psychotherapy cases are allocated to junior doctors following assessment by the consultant psychotherapist in the service.
The post-holder will maintain consultant responsibility for psychotherapy cases throughout the psychotherapy treatments.
Junior doctors are supervised in supervision groups typically made up of five supervisees for one hour a week.
Psychotherapy supervision consists in the discussion and the review of the clinical management of individual psychotherapy cases.
6. Attending weekly and fully participating in the Westminster North and South Hub multidisciplinary team meetings
7. Direct involvement in the community teams’ clinical work alongside other hub consultant colleagues.
8. Providing clinical leadership to allow for the provision of a full range of psychological treatments across Westminster.
9. Medical input and clinical leadership in line with the Trust’s strategic objectives.
TEACHING:
1. Supervision of psychotherapy cases of junior doctors during their rotation placement in Westminster services. Psychotherapy cases are allocated to junior doctors following assessment by the consultant in the service.
The post-holder will also be responsible for completing the relevant ARCP assessments. This includes Structured Assessment of Psychotherapy Experience (SAPEs) for his/her CT, ST doctor-supervisees and Psychotherapy Assessments of Clinical Experience (PACEs) where needed once every six months.
2. Balint Group sessions are held on a weekly basis for junior doctors based in KCW.
The post-holder will complete the relevant work-place based assessment for ARCP purposes. The post holder can work together with Dr Florence Dalton, Medical psychotherapist in K&C and the Trust psychotherapy tutor to divide responsibilities for running the Balint group across KCW.
3. Regular teaching in adult psychotherapy for the Combined Charing Cross and St Mary’s MRCPsych course.
4. Active role in the Westminster, Kensington & Chelsea postgraduate teaching programme provided at St Charles Hospital.
5. Teaching of any medical students and Foundation Year doctors in the N and S hubs as relevant.
6. Consultant psychiatrists are also required to provide Educational supervision responsibilities for Scheme trainee doctors (CTs and/or STs)and the post holder is expected to provide this for 2 trainees.
