Transitional care and flow Non registered band 5 role.
The Transitional Care Flow non registered band 5 role is an exciting new role to work as an LCHS representative supporting all aspects of discharge and admission to the beds commissioned with LCHS alongside the services within the Urgent and Emergency care division.
This main function of this role is to have the oversight of the band 4, 3s, and 2s within the transitional care team across the service along with having the ability to work clinically when needed to support the different services, completing assessments when needed.
The successful candidate will be working with the Transfer of care practitioners and Discharge to Assess to align our services within the Group model, servicing the county and continuing to provide excellent care to our patients, closer to home. They must be able to work with daily challenges in a manner that reflects the LCHS Way while working for the best outcome for the patient finding innovative solutions to any issues.

A 7 DAY WEEK SERVICE 08:00-20:00
BASE- To be based predominantly within Beech house, some county wide work maybe required.
We have an exciting opportunity within our Transitional Care and Flow Team for a dynamic, engaged, person to lead our Flow team as well as being a face to face role that supports our patients in their care journey. The role also involves working alongside registered and unregistered multidisciplinary professionals and also encompasses compiling reports and wider system calls and meetings.
The role requires a candidate good communication skills, computer skills and is familiar with SystmOne.
You will need to show that you are a team player supporting your service and willing to be adaptable as well as supporting the group model. You will also be able to work independently and within a timeframe to ensure that pathway processes are followed, as well as leading a team.

*Working within an Multi-disciplinary team predominantly based in Beech house, Lincoln with the ability to travel county wide with the aim to effectively, safely and timely admit patients to the services within the LCHS portfolio.
*Leading a team within transitional care and flow co-ordinators, completing appraisals, one 2 ones and building stakeholder relationships to aid the staff to give the best service.
*Being an innovative individual who thrives on new challenges, ensuring that the best is always aimed for and learning, while striving to achieve care close to home.
