Rehabilitation Assistant Band 3 – Full Time

Base: Lincoln

Are you passionate about delivering a high quality service to patients who have had a stroke in Lincolnshire? Are you motivated and dedicated to supporting patients with the transition from hospital to home, and to continue with their rehabilitation in the community? The service has some exciting new developments happening across the county and we are looking to recruit highly motivated Rehabilitation Assistants to join the service in these exciting new developments.

The innovative Stroke Service provides high quality specialist stroke rehabilitation in the community across Lincolnshire. The service has four bases across the county and works as a very flexible team.  Your post will require the ability to travel to meet the needs of this county wide service and applicants must be able to work on a rota basis to assist in covering a 7 day a week 8am to 6pm service.

Main duties of the job

You will play a key role in supporting Therapy staff in delivering high quality patient centred care and rehabilitation to people who have recently experienced a stroke, in their own homes as well as community settings. You will be working with a full multidisciplinary team of specialist physiotherapists, speech and language therapists, occupational therapists, clinical psychologist, psychology assistant, assistant practitioners and rehabilitation assistants.

It is an exciting time to work in stroke rehabilitation in Lincolnshire. Our service has recently expanded to include Clinical Neuropsychology and we will soon look to recruit a Community Dietitian.  You will also work closely with colleagues in the wider system to facilitate seamless service provision to support patient’s recovery following Stroke.

Detailed job description and main responsibilities

To work as an integrated member of the Lincolnshire Stroke Service delivering person centred care, assisting Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists and the wider multidisciplinary team with patient interventions.

To deliver clinical interventions defined within individualised patient care plans, as directed by the senior staff.

To maintain high quality standards in all aspects of care delivery, working in partnership with primary care colleagues, other health professionals, patients, carers and the statutory, independent and voluntary organisations.

Qualification

Essential criteria
  • Good general education
  • Health related NVQ level 3 or equivalent
  • 3 GCSEs grade C including Maths and English or equivalent
Desirable criteria
  • European Computer Driving licence
  • Food hygiene certificate

Experience

  • Experience within caring environment
  • Demonstrates attitude and aptitude needed for the post
  • Experience of community working
