Job overview

To undertake a multifunctional role combining an extensive range of administration, secretarial and support duties for the Senior Managers of the Home First Team. This includes the booking of Home First discharges from within East Lancs Hospitals, which includes our peripheral sites. The postholder will provide a discrete, courteous and efficient first point of contact for all communication within the team.  Prioritise and organise own workload within the Hub. Provide cover for the Transport Co-ordinator for annual leave and sickness.

Main duties of the job

  • Work predominantly at BGH but be  flexible across all ELHT sites to meet the needs of all discharges and transfers of care for all adult patients.
  • The post holder will be expected to utilise several IT based systems
  • To have the in-depth knowledge and skills to act as first response to ward staff at all levels and other multi-disciplinary professionals ensuring there is a consistent and informed approach in relation to effective and timely discharge planning under the guidance of the Business Support Manager
  • To promote and maintain effective communication channels between patients, their relatives and relevant teams too successfully coordinate discharges.
  • Be a point of contact for all level of queries with regards to discharge and flow within remit of knowledge and role. Escalate unresolved issues, complaints, disputes or delays in the care process to the relevant senior team leader.

  • The post holder will be required to maintain a confidential and professional approach at all times. The role will involve communicating with patients, relatives, colleagues, nursing and AHP professionals across health and social care, either by telephone or face to face.

Essential criteria
  • IT Skills - experience with working with all Microsoft systems
  • Business Administration qualification NVQ or equivalent experience
  • Ability to manage own workload
Desirable criteria
  • Previous experience in a Healthcare setting

  • Knowledge of discharge planning processes and pathways.
  • Excellent organisational skills
  • Proven ability to maintain confidentiality
  • Ability to prioritise an unpredictable workload and work to time scales in a fast past environment
