CLCH BRENT RAPID RESPONSE - SENIOR PHYSIOTHERAPIST
We are seeking an enthusiastic Specialist Physiotherapist to join our Unplanned Care team based at Sudbury Primary Care at Wembley, London delivering care to patients closer to home and preventing unnecessary hospital admission in Brent.
This role will primarily work under the Rapid Response pathway, but may require the candidate to work across the other pathways when required.
All pathways deliver nursing and therapy care in a patient’s usual home environment.

RR focuses on preventing avoidable hospital admissions for patients with acute ill-health conditions by providing holistic assessment within 2 hours of referral and treatments over a 5 day pathway.
The pathway is delivered in conjunction with Adult Social Care to support the patient’s reablement.
The Unplanned Care team works in an integrated way with the other community providers in Brent to ensure holistic support is provided.

The successful candidate needs to be a registered Physiotherapist, ideally with advanced clinical skills, significant knowledge and experience in the clinical care of adults and older people in a range of healthcare settings.
Candidates should be self-motivated with strong organisational skills and able to make clinical decisions autonomously as required.
As a clinician you will use your clinical expertise, levels of judgement, discretion and decision making to support patients at home, improve patient outcomes and reduce likelihood of unnecessary admissions to hospital.
You will be required to complete holistic assessments, giving appropriate interventions as required and have knowledge of community services to plan the on-going care of patients.

- Current HCPC registration
- Evidence of up to date CPD portfolio
- Involvement in Appraisal Scheme
- Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People , Rehabilitation
- Member of Professional Body

- Member of a Special Interest Group relevant to this post
Experience

- Experience post registration as a Band 5 physiotherapist
- Experience working at a Junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory

Skills and Knowledge

- Skills in the assessment and treatment of adults with complex multi-pathologies
- Knowledge and understanding of complex medical conditions including multipathologies and falls Knowledge of role of Allied Health Professionals and interdisciplinary team working
- Ability to apply core Physiotherapy skills across a range of clinical conditions within a client centered, goal setting
- Knowledge of outcome measures and evidence-based practice
- Ability to problem solve, prioritise and demonstrate clinical reasoning
- Good communication and report writing skills, in both verbal and written format
- Basic IT skills – Microsoft Word, Power point, Excel
- Teaching of junior staff / students, peers in Inservice training setting
- Knowledge and understanding of the role of Social Services and voluntary agencies
