An exciting opportunity has arisen in South for a Senior physiotherapist to join our Diss Community Therapy service. You will work with and support a team of friendly, approachable Physiotherapists, Occupational Therapists and Community Assistant Practitioners. The post will be based at Diss Health Centre and requires travelling out to see patients in their own homes or in care homes in the surrounding villages.
You will deliver high quality physiotherapeutic interventions and health care to patients within a variety of community settings, which may include in-reaching on to the in-patient unit as needed. To provide proactive, holistic case management and rehabilitation whilst working with patients to help them set realistic goals. Utilising health coaching skills to actively engage with patient and their family to ensure that expectations are appropriately managed.

You will assess and prescribe exercise programmes to patients with, but not exclusive to, frail and elderly, palliative care and rehabilitation needs, to achieve quality care and independence where possible.
You work with the integrated team to facilitate early discharge from hospital, and also to prevent unnecessary admission to hospital.
You will be working with health care professions , statutory and non-statutory agencies to provide a seamless integrated service to our service users.
You will provide advise, support and supervision to junior staff, therapy assistants and students.

Qualifications

- Diploma or 1st level degree in Physiotherapy, or equivalent experience
- Current HCPC registration
- Clinical educators course

- Be able to kneel, bend & stoop, and work in cramped environments.
- Manoeuvre limbs of around 5-6kg
- Manoeuvre patients using handling aids
- Must have a full UK driving license and have access to a vehicle
- Able to communicate effectively in written and verbal English Language
- Be reliable
