An exciting opportunity has arisen for a suitably qualified and enthusiastic physiotherapist or exercise physiologist to join this well established cardiac rehabilitation service as the Exercise Lead. The post holder will join the multidisciplinary team to contribute to the delivery of cardiac rehabilitation care in a variety of settings. The role will involve leading on and implementing cardiac rehabilitation interventions in Flintshire and Wrexham with requirements to travel across Wrexham and Flintshire.

The client caseload includes those following acute coronary events and /or coronary intervention and those with heart failure. The postholder will promote independence, confidence and improved quality of life for both the client and their family where necessary. You will require excellent communication and motivational skills and be able to work effectively both within a team and on your own.

This post will be suitable for someone with relevent experience in cardiology and/or cardiac rehabilitation. You are strongly advised to read the Job Person Specifications for this role and apply knowing you fulfull these essential qualities.

Part time / Full time

Flexible between Monday to Friday, between the hours of 8.30am to 5pm dependent upon travel and needs of the service

Base: either Wrexham Maelor Hospital or Connah’s Quay Health Centre, with travel across Wrexham and Flintshire.

Main duties of the job

THIS POST IS FIXED TERM/SECONDMENT FOR 12 MONTHS TO MEET THE NEEDS OF THE SERVICE

• Working collaboratively with the advanced practitioner to deliver a high quality clinical service in the East Integrated Health Community.
• The assessment and management of complex patients. Developing and delivering individualised treatment programmes and working effectively with the wider multidisciplinary team.
• Providing clinical leadership and day-to-day management of the service including planning and coordinating service delivery and service evaluation once competencies completed.
• Supporting the on-going education and development of the team to ensure a high quality, evidence based service.
• Lead educator for student placements.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
