Senior Children’s Manager - Continuing Healthcare 

Agenda for Change Band 8a, Full Time / Permanent role

Based in Acton House, Worcester or St Owen's, Hereford.

HWICB are seeking an experienced, highly motivated, flexible and multi-skilled Children’s Nurse/ associated Allied Health Professional/ Social Care Practitioner with excellent communication and organisational skills as the new Senior Children's Manager for Children’s Continuing Care (CCC).

This role is a vital part of the Children’s Continuing Care Team and an integral part of Continuing Healthcare (CHC) within the ICB.

The successful candidate will be a Senior Clinician/AHP/ Social Worker with significant experience of complex children’s care and will act as line manager for a small team.  Working together with multi-agencies, stakeholders/ other professionals across both the public and private sector, the post holder will ensure a ‘team around the child’ ethos is developed and maintained for all children/young people who require support for their complex needs and are CCC eligible within Herefordshire and Worcestershire.

Main duties of the job

The post holder will need to demonstrate an integral understanding of how the processes of Continuing Care and Personalised Commissioning work together- and provide leadership and vision in developing the service and increasing the number of personal health budgets for children and young people in Herefordshire and Worcestershire.

The post holder will be required to provide assurance that agreed CCC processes and pathways are delivered as outlined in the NHS National Children’s Continuing Care Framework. They will be required to oversee localised arrangements for complex cases- facilitating joined-up service delivery with local authority/ educational authorities across Herefordshire and Worcestershire. This covers assessment, arrangement, procurement and monitoring of individual children/ young people individualised packages of care within the community.

If you are a nurse, you will be registered with the Nursing and Midwifery Council as a Registered Children’s Nurse RN Child/ RSCN. If you are an AHP or Social Worker you will be educated to a degree level or equivalent and, in all cases, you will have appropriate experience in the provision of care for children with complex needs and Children’s Continuing Care.

Driver’s licence and access to own vehicle with business insurance is essential.
