Great Western Hospitals NHS Foundation Trust’s services are forward thinking and are actively developing community health services in line with the national agenda. You will be at the forefront of developing innovative quality services delivered seven days a week.

We are increasing our well-established services provided to local people and as a result we have been expanding our team of qualified therapists and support staff.

We are an integrated, hard-working but highly rewarding multi-disciplinary team, who aim to make a real difference to people’s lives. We are looking for an experienced, flexible and enthusiastic Physiotherapist who would relish the opportunity to join our well established inter-disciplinary team. You will with previous NHS experience who would relish the opportunity to join our well established inter-disciplinary team.  You will have a desire to develop your skills in promoting health, well-being and independence across our community rehabilitation services. 

You will need to be a car driver with access to your own vehicle - independent means of travel is essential for this role.

 

Prif ddyletswyddau'r swydd

  • To be professionally and legally accountable for all aspects of own work, including the management of patients/clients on your caseload.
  •  Use highly developed specialist skills to screen and prioritise referrals for home based and venue based rehabilitation services and refer to other services when necessary.
  •  Assess the impact of the home environment and patient/client’s social network which may influence the treatment plan and present a risk to the personal safety of the therapist.

There is the opportunity to rotate within the services that we offer as a community rehab team including reablement and Homefirst, community discharge to assess beds, Parkinson's Disease, Specialist falls and vestibular service, homebased  rehabilitation service and Urgent (Rapid) Community response.

Some of these are clinic based service however majority of these patients are seen at home within the Local Swindon area covering approx 12 miles end to end. 

There are training and development opportunities within the team to support your personal professional goals. We work closely with the GWH hospital to facilitate hospital discharges as we are the same trust. 

You will be joining a very friendly,  social  and supportive team  with a wealth of experience in a variety of areas which the team is happy to share.

  • Be the physiotherapy lead within the inter-disciplinary team, providing highly specialist advice within Community Health Services and externally to Consultants, GP’s, members of the primary and 
    secondary health care teams, Social Services, voluntary agencies and others as necessary.
  • Take a lead role in audit and other clinical governance activities.
  • Act as a source of highly specialist advice and teach support workers and students from other professions.
  • Clinically supervise, advise, support and teach rehab assistants, physiotherapy students and less experienced physiotherapists

Qualifications

  • HCPC Registered
  • Evidence of postgraduate learning relevant to clinical area – e.g. Neurology, Falls, Elderly rehabilitation, use of equipment.

  • CSP membership
  • Post graduate training e.g. Normal movement training, vestibular rehab
  • Apprenticeship / NVQ Assessor

Experience

  • Experience of physiotherapy with broad rotational experience
  • Experience of supervising and training staff
  • Experience of writing reports/ discharge summaries containing complex information
  • Experience of working in the community as a lone practitioner
  • Experience of prioritising work/caseloads and to respond to unpredictable work patterns
  • Previous NHS experience
