Job overview

To deliver a high standard of health care, supporting Exercise therapist, Physiotherapist and Cardiac rehabilitation Nurses, together with the wider MDT, to assess, plan and implement a structured evidence-based exercise and education programme to service users with acute and chronic cardiac conditions across Swindon. 

To manage and organise the flow of patients through phase 3 of the cardiac rehabilitation programme, evaluating and adapting interventions, informing the team of any changes or developments in patient’s progress in a timely manner. 

To aid rehabilitation and improve functional ability, education, self-management and confidence of patients.

Main duties of the job

  • To deliver a structured evidence-based exercise programme as screened and delegated by a qualified therapists in line with agreed local pathways and national targets.
  • To manage and prioritise own clinical workload.
  • To lead cardiac rehabilitation exercise classes for delegated patients. 
  • Work in partnership with the specialist nurses and exercise therapist in supporting patients’ to achieve their tailor made, person centred goals reviewing and adjusting accordingly.
  • To help with the delivery of the physical activity component of the programme.
  • To assist with sub-maximal exercise assessments for a range of cardiac conditions.
  • To work as part of a team to directly deliver and promote physical activity and lifestyle interventions to adults within community setting as required.
  • Accurately record and document the delivery of care.
  • Maintain equipment and resources, required to support physical activity programmes.
  • Participate in the effective reporting of feedback, incidents and complaints. This includes submission of appropriate documentation to facilitate the investigation process

Clinical

Person specification

Qualifications

Essential criteria
  • NVQ Level 3 study (Health and Social care) or evidence of level 3 study or evidence of extensive clinical experience in a health and social care focussing on cardiac patients.
  • GCSE Maths and English or Maths and English – level 2 functional skills
Desirable criteria
  • BACPR Specialist Exercise Instructor Level 4 Cardiac qualification.

Experience

Essential criteria
  • Experience of leading structured and graded exercise sessions including high risk cardiovascular patients.
  • Experience of leading / developing relevant programmes of lifestyle interventions for high-risk clinical groups.
Desirable criteria
  • Experience of data collection and analysis.
  • Participation in research/audit.
  • Experience in adapting exercise to those with co-morbidities.
  • Evaluating the outcomes of cardiac rehab.

knowledge

Essential criteria
  • Good organisational and time management skills including management of a delegated case load.
  • IT and nummary skills.
  • Team player, flexible, adaptable
  • Be able to work efficiently alongside all members of the MDT
Desirable criteria
  • Up to date knowledge of national and local strategies to reduce cardiovascular disease.Experience in adapting exercise to those with co-morbidities.
  • Knowledge of BACPR and ACPICR standards
