Job overview
This an exciting importunity to join a new and evolving team. Centered around the early identification, recognition,assessment and treatment of a deteriorating patient across CHFT
The post holder will be a member of the Resilience, Acute Flow and Transformation directorate sitting within the Corporate Division, working effectively with fellow ward teams and Divisional Matrons to deliver quality services for patients, efficiently. There is a specific focus on the deteriorating patient and improving outcomes for patients. As part of the interview process, all candidates will have an Objective Structured Clinical Examination (OSCE) assessment in addition to the Panel interview.
The post holder will be responsible for the assessment, planning, implementation and evaluation of care to acutely deteriorating and critically ill patients. This includes non-medical prescribing, ultrasound guided cannulation, arterial blood gas sampling, spirometry out of hours, review of all tracheostomies/laryngectomy patients, liaising with Critical care.
- To provide clinical and managerial leadership for nursing and other healthcare professionals.
- The post holder will provide advice and support to wider members of the multi-disciplinary team.
- To facilitate the stepdown of patients from Critical Care to ward areas.
- To provide informal and formal education to all healthcare professionals within the Trust.
- To proactively manage the clinical workload at night as necessary
Main duties of the job
- The post holder is responsible for leading the coordinated response team to ensure safe and effective patient care.
- To assess, plan, implement and evaluate care to deteriorating patients referred by the ward teams and NEWS2 escalation.
- Work closely with the medical, surgical and critical care registrars and junior teams, attending medical handovers.
- Provide expert clinical advice and guidance to ward staff in the support of patients 24/7.
- To provide informal and formal education to all staff regarding recognition and care of deteriorating patients and acutely ill patients.
- Manage and support the Nursing associates triage request via the out of hours Management system to coordinate the delivery of care in Out of Hours period.
- Establish and maintain effective communication with all members of the multi-disciplinary teams.
- To liaise and work closely with medical and nursing staff within Critical Care.
- Work in partnership with ward managers to ensure that prescribed standards of patient care are met.
- To support the Clinical lead with Quality Improvement projects across the trust, taking the lead when required.
- To provide relevant and clear information, education and advice for patients, carer’s and family at all times.

Detailed job description and main responsibilities
- Initiate, lead and coordinate management of patients identified as ‘clinically at risk’, referring to appropriate colleagues when necessary.
- Ensure timely and appropriate care is delivered by the relevant health care professionals.
- To undertake a comprehensive assessment of patients, participate in the assessing, planning, implementation and evaluation of individualised programmes of care.
- Co-ordinate clinical emergencies i.e. cardiac arrest.
- Act as an innovative and enthusiastic role model, providing leadership, guidance and advice to staff on clinical, operational and professional issues.
- Demonstrates clinical expertise by role modelling patient centred practice.
- Ensures that staff adhere to the systems and processes in place to support the provision of patient centred care to agreed standards.
- Ensures adherence to the CHFT and NMC guidelines for the safe handling, administration, storage and custody of medicinal products
- Ensures good communication between staff and relatives is maintained to assist in their understanding of care, treatment and progress of the patient is and implemented and adhered to.
- Maintains own personal and professional development in accordance with NMC Code of Professional Conduct and within the Scope of Professional Practice.
- Participates effectively in the multi-professional team providing an expert nursing perspective.
- Contribute to the audit process in line with the National Outreach Forum Standards and utilise results in practice.
- Ensures staff are aware of and work in line with Trust policies.
- Support patient care using technology relating to task management, observations and handover.

KNOWLEDGE, EXPERIENCE & EXPERTISE
Essential criteria
- Extensive & recent management and leadership experience in acute/critical care setting
- Advanced acute clinical knowledge
- Ability to coordinate workload and prioritise workload
- Possess excellent communication skills and able to work within MDT
COMMUNICATION AND RELATIONSHIPS (INCLUDING MANAGEMENT RESPONSIBILITIES)
Essential criteria
- Excellent written and verbal communication skills.
- Understands complex clinical and technical problems and exercises specialist clinical judgement when assessing and interpreting specialist and acute patient conditions
- Competent in use of Nerve centre/ willingness to undertake
- Has self -confidence and uses this to lead, involve and challenge others as appropriate
- Displays a strong motivation and commitment to success
QUALIFICATIONS / TRAINING
Essential criteria
- Independent Prescribing or to complete this in within 2 yrs.
- First level registration & current NMC registration
- Evidence of post registration study and development in acute or critical care
- Teaching and assessing post registration qualification
- PG- Certification in acute care/Critical care course or willingness to do this within 2 years
- Master’s Level Qualification / working towards. Or equivalent highly specialised experience & Knowledge
- National outreach forum competencies to enhanced level. Or working towards
- ALS certificate (or willingness to immediately undertake)
- IRMER (Ionising Radiation), willingness to undertake to enable x-ray requesting
- Evidence of leadership skills
- Evidence of mandatory training
- Competent in cannulation & phlebotomy or willingness to undertake in 3 months including ultrasound guided cannulation
- Competent in male/female catheterisation or willingness to undertake in 3 months
- Competent in arterial blood gas sampling or willingness to undertake in 3 months
- CVAD trained competent/willingness to complete within 3 months
- Verification of Death or willingness to undertake in 3 months
Desirable criteria
- Competent in spirometry/ willingness to undertake within 3 months
