Following the exciting development of our Multispecialty Virtual Ward in conjunction with our partners within Bradford District and Craven we have a number of opportunities for roles within the service.
The Virtual Ward is an innovative service and the aim to is support people to receive care, support, and interventions within their own home, this improves the quality of care for the individuals, reduces the length of time people stay in hospital, reduces the need for people to be admitted to hospital and ensure beds are available to treat the acute admissions.

This is an opportunity to gain experience and shape the development of the new service for the future. The role will involve working as part of a multidisciplinary team both within secondary care and the community providing care at home.
The successful applicant will encompass specific responsibility for assessing, planning, implementing, and evaluating nursing care and expertise within a community setting. The post holder will help lead and coordinate the clinical, managerial, and educational requirements of the clinical area ensuring the highest standards of nursing practice and care are delivered as reflected by the Virtual Ward. They will ensure the safety and wellbeing of service users and work in a manner that promotes dignity and human rights through the adoption of person-centred care principles. The post holder will also deputise for the Virtual Services Team Manager and act as a role model for junior team members in the delivery of a high standard of nursing care. A full Driving license is essential for this role.

- Significant Community / Ward based experience caring for patients with complex needs.
- Experience of teaching / assessing / supervising others in a clinical setting.

- Experience of using Systm one / BTHFT IT systems.
- Proven experience on managing a team, including management of rotas, managing competencies, resources and staff.
Skills

- Good communication skills and ability to deal with sensitive information.
- Demonstrates ability to influence and negotiate with others.
- Able to assess plan, implement and evaluate programmes of care.
- Able to work as an effective team member.
- Able to manage difficult situations.
- Basic computer skills including ability to use Microsoft Word and Electronic Patient Records.

Qualifications

- Registered Nurse
- BSc / 1st degree (health related) or equivalent qualification / training / experience.
- Mentorship / teaching qualification.
- Evidence of continuing professional development.
