Job overview

This is an exciting opportunity for an experienced cardiology or long-term conditions nurse to develop specialist skills in the management of patients with heart failure. We are looking for a pioneering, dynamic and forward thinking nurse who is passionate about providing holistic care and support to patients with both a new and existing diagnosis of heart failure. Candidates should have high clinical standards and be committed to quality patient care. The successful candidate will work independently but will also be part of the existing community heart failure team to identify and support patients living with heart failure, monitoring outcomes to improve quality of life, prognosis and reducing re admission rates. 

 

The role will involve working across out-patient settings and home visits, undertaking patient assessment, implementing, and devising patient management plans following an approved local guideline. You will be required to be involved in providing patient / staff education, training, clinical supervision, and service development as part of this role.

Main duties of the job

·         To support and facilitate the improvement of care and experience for patients with heart failure.

·         Provide evidence based individualised care for heart failure patients and their carers’, which empowers patients to take an active role in their care.

·         To work with others to reduce the need for unplanned admissions to hospital and /or the need for prolonged stay in hospital.

·         To contribute in the development and delivery of national and local priorities for heart failure, working collaboratively with other cardiac nurse specialists through cross boundary working.

·         To work autonomously assessing individual patient needs, initiating investigations and utilising advanced skills and knowledge  to initiate and/or titrate heart failure medication and monitor outcomes to improve quality of life, prognosis and reduce re admission rate of heart failure patients.

·         To act as a resource for all members of the multidisciplinary team and provide specialist advice concerning aspects of heart failure management.

·         To contribute in developing and delivering education and training to multidisciplinary teams and all service users. 

·         To exercise accountability set out in the NMC Code

Experience

Essential criteria
  • Significant cardiology experience or experience of long-term condition
  • Experience of heart failure case management or management of a long-term condition
  • Knowledge and experience of handling complex relationships and interpersonal dynamics
  • Able to support learning and education for patients and practitioners.
  • Experience of cross boundary MDT working
Desirable criteria
  • Experience of leading service development
  • Palliative Care Experience
