We are developing an integrated community model, with the focus on new ways of working within integrated mental health teams to support people living in their communities with mental health challenges.
Main duties of the job
You will be a Registered Mental Health Nurse, with experience of working in community settings with Service Users with complex mental health and substance misuse issues.
You will be responsible for a caseload of clients providing clinical care and guidance for those with drug and alcohol dependency and to support outreach to substance misuse services.
Experience in assessing and formulating individual intervention plans and outcome measures for adults with complex mental health problems and have current knowledge of NHS policies and practices in mental health. You will participate and support the CMHT duty system and be responsible for managing the mental health needs of Services Users referred to the team.
Successful applicants will have previous knowledge of the recovery agenda and experience of working with a focus on resilience and confidence based prevention to promote better outcomes for individuals. Have a working knowledge of psychotropic medication and effectiveness plus the ability to deliver a range of therapeutic interventions. In addition, the post holder will demonstrate the ability to undertake and manage clinical risks and develop robust clinical management plans for Service Users with the service.
You will support the Team Manager and Clinical Lead to adhere to and deliver high quality service and have in depth knowledge of national quality indicators.

Experience
Essential criteria
- NMC - RMN Registration
- Experience of working within mental health services
- Experience of assessment of people with substance misuse and related problems
- Experience of working in multidisciplinary team
- Knowledge of NHS issues and current policies and practices in mental health
- Evidence of commitment to promoting service users and carer values
Desirable criteria
- Higher qualification at least to degree level in relevant subject
- Post graduate training relevant to Mental Health
- Recent and relevant experience of working with clients experiencing issues with substance misuse
- Evidence of working with complex mental health and substance misuse service users within inpatient and/or community setting

You must have appropriate UK professional registration.
