The job holder will operate within the Enhanced Community Mental Health Team alongside all other Secondary Mental health services such and hold responsibility for meeting the community mental health needs of Dudley CCG GP practices.

The ECMHT provides a service accessed via a single point of entry via Duty.  All referrals are subject to initial screening for appropriateness and priority and are forwarded for individual assessment of mental health needs.  Appropriate intervention is implemented based on identified needs.  

The job holder will operate from a community-based facility and will provide service from either the work base or their home, via the telephone or within the service user’s home.  The job holder may also operate from other health care settings including hospitals and primary care surgeries or clinics.

The ECMHT currently operates a service 7 Days per week between 9-5 hours but is expected to advise service users and carers of access to services outside of these hours and liaises and refer on to other mental health care providers in cases where support outside of these hours is required.

To take the specialist lead role in the clinical management of people over 65 years of age and people of working age with dementia with complex mental health needs. 

 

To plan, deliver and evaluate interventions and treatments where there are complex issues and/or serious illness.

 

To assess, develop and maintain high standards of care appertaining to the mental and physical well-being of this specialist client group, providing safe co-ordination, care, treatment and advice.

 

Administration and monitoring of medication to facilitate compliance and adherence to treatment.

To take the specialist lead role in the clinical management of people over 65 years of age and people of working age with dementia with complex mental health needs.  To plan, deliver and evaluate interventions and treatments where there are complex issues and/or serious illness.

 

To receive, screen and assess all referrals (via an open referral system) that come into the service and undertake a comprehensive mental health assessment, risk assessment and carers’ assessment on a regular basis.  To interpret and respond to a range of complex and arising issues applying appropriate decision making and interventions.

 

To act as a Consultant to patients, carers and other professionals.  To take sole responsibility and be accountable for the mental health care for individuals not know to the Consultant Psychiatrist.

 

All qualified specialist mental health nurses working with Older Adults are autonomous and independent practitioners, responsible for their own work and interventions and take full responsibility for working with even the most complex presentation.  To assess, develop and maintain high standards of care appertaining to the mental and physical well-being of this specialist client group, providing safe co-ordination, care, treatment and advice.

 

To assess physiological and psychological functioning where there are multiple health complexities and poly-pharmacy.

 

To liaise with statutory / non statutory / voluntary agencies in planning, assessing and evaluating programmes of care.

 

To provide professional and clinical services, taking responsibility for the planning, management and co-ordination of 24 hour care of patients.

 

In the absence of the provision of any Psychology service to older adults with mental health problems, to provide psychological therapies.

 

To undertake a management role, both in individual localities and on behalf of the Borough-wide team, in line with those of a team co-ordinator, as there is no Team Co-ordinator in the management structure of the older adult service. To participate in relevant training appropriate to this extended role.

 

Community mental health practitioners are based in five different localities and the existing service operates Monday to Friday; 9:00 am – 5:00 pm.  The service can be accessed by clients and carers directly and referrals are received from general practitioners, psychiatrist and all other health and social care professionals.

 

The main working environment will be the patient’s own home and / or other areas in the community
