Trosolwg o'r swydd

The post holder will be part of a team of clinicians whose responsibility it is to develop and improve services for people with common mental health problems in primary care, attended integrated care team meetings when required. Post holders will have high level of autonomy within their specific primary care setting, developing and managing all aspects of their service according to local need and within the parameters of the service operational policy.

Team members will actively participate in and support the development of new ways of working which will strengthen and support the co-ordination of a whole systems approach for response to identified client need. Members will also be involved in regular service evaluation, audit and research initiatives.

To assess the suitability of counselling for a range of common mental health conditions

To assess and deliver evidence based counselling to individuals presenting with complex mental health problems

To provide individual assessment and treatment, integrating a broad range of counselling models

To facilitate and co-facilitate groups agreed within the service

To offer counselling to adults with mild to moderate mental health problems who can present as distressed individuals with difficult and varied circumstances ranging from bereavement, loss and other life events.

There is a frequent requirement for intense concentration requiring continuous re-evaluation and assessment of internal and external processes, and the formulation of appropriate therapeutic interventions

There is a requirement to plan and organise case management, appropriate interventions and liaison with other healthcare professionals

To liaise with appropriate healthcare staff, including referrers, carers and family members

To develop links with primary care and social services teams, attending practice and primary care team meetings and home visits when required

To attend and/or deliver supervision as per Trust policy

To manage own caseload, prioritising referrals as appropriate and providing feedback to Lead Counsellor on current waiting times

We are looking for a dynamic primary care mental health counsellor to join our Primary Care Mental Health Service. We are looking for an experienced Counsellor trained in psychodynamic, person centred or an integrative counselling modality to complement our team transforming care for patients.

To perform in accordance with BACP Ethics and Guidelines and NHS local policy

To utilise specialist knowledge of psychological frameworks and application of theories based on degree, post graduate training and experience

To liaise with GP’s and other primary care staff and other teams and agencies attending Integrated care team meetings when required

To clinically supervise trainees, other health professionals and colleagues

To participate in Continuing Professional Development and other developmental activities

Qualifications/ Training

  • Recognised counselling qualification (level 5 or above with significant relevant post qualification experience), specifically psychodynamic, person centred and / or integrative
  • To currently be an accredited practitioner or working towards completion of accreditation with a professional body i.e. BACP, HPC, UKCP
  • Evidence of continued professional development (CPD)
  • Training in risk assessment/willing to undertake

Experience

  • Experience of working within Primary Care
  • Experience of working post Qualification in a GP/NHS mental health setting
  • Knowledge of Risk Assessment/Risk Management
  • Ability to identify common mental health conditions as determined by ICD10
  • Knowledge of appropriate agencies for client referral
