Inflammatory Bowel Disease Clinical Nurse Specialist

Probationary Period

All new staff to the Trust will be subject to a standard 6 month probationary period, the details of which will duly follow in your contract of employment.

Agile and Flexible Working

All our staff will have a work base. However the Trust operates as an Agile Working Team therefore staff may be working from home on a regular basis as required by the service. However, occasionally travel around Warwickshire and into your base will be required so access to your own transport would be helpful.

A fantastic opportunity has arisen in South Warwickshire for a well-motivated and experienced nurse to join the Gastroenterology nursing team at Warwick Hospital. 

The successful candidate will work as an IBD Clinical Nurse Specialist within the Trust based at Warwick Hospital but may be expected to work across all Trust sites as required.

The post holder will be expected to provide evidence-based care, keep up to date with latest research and their own education. This will enable them to support inpatients and outpatients requiring IBD expertise.

The post holder will have at least 3 years post registration experience, and with IBD experience. Must be willing to undertake IBD specific training and qualifications as necessary.

The post holder will work as a Specialist Nurse within the Department of
Gastroenterology providing information, education, care and support to patients. The post holder will be expected to collaborate with colleagues to provide a seamless service for patients. The post holder will work collaboratively and liaise with other health professionals across the local health economy to ensure there is efficient clinical management for these patients. The post holder will work with other members of the clinical nursing team and as part of the multidisciplinary team providing support for patients with Inflammatory Bowel disease.

Knowledge skills and experience required:
Be a first level registered nurse with 3 years experience

A minimum of 2 years experience in gastroenterology or an acute medical
setting

Hold a Diploma/Degree in Professional Nursing

Possess experience of patients with Inflammatory Bowel Disease

Provide evidence of continuous personal and professional development

Possess the aptitude and confidence for working within a rapidly changing
environment

Have excellent communication skills both verbal and written

Have excellent interpersonal and organisational skills able to manage own
workload

Key Responsibilities
The post holder will see patients with IBD in clinic with support from other
members of the team

Review patients in clinic and monitor and supervise their clinical care

Provide clinical advice to patients, carers or members of the multidisciplinary team

Provide clinical support to patients via the telephone help line

Liaise with other members of the healthcare team and other agencies as
required

Provide CNS support to newly diagnosed patients within the hospital and in
Endoscopy when they are told their diagnosis.

Education

Provide education for patients about their care & treatment

With the multidisciplinary team, develop and update policies that affect
delivery of care to patients

Identify the learning needs of colleagues and facilitate/provide formal and
informal teaching sessions for health care staff.

Be aware of current clinical research and disseminate relevant findings to
appropriate health care professionals

Demonstrate responsibility for own personal and professional development by identifying own experiential and educational needs and pursuing training and development through Personal Development Planning.

Management/leadership

Manage own caseload of patients seeking support from the members of the
clinical nurse team and multi disciplinary team.

Act as a clinical leader and role model sharing expertise with colleagues.

Ensure clinical procedures and protocols are adhered to.

Promote evidence-based practice, participate in multidisciplinary audit and
make recommendations for service development as appropriate.

Participate in standard setting and monitoring patient care, benchmarking the department’s performance against best practice.

Develop and update guidelines with other members of the team for best
practice in the Trust Communications and Working Relationships:

Ensure that timely and accurate records of any patient/carer interaction are recorded in the patient’s healthcare records.

Responsible for data input to the patient management systems.

Effective communication with all members of the multi-disciplinary team
regarding care of patients.

Ensure that patients and their carers know how to contact the department and have contact numbers.

Provide information, education and support to patients, carers and other
health care professionals

To liaise with other agencies and professionals to maintain good service
provision.
