Job overview

Full time 37.5 HRS - Working out of a variety of bases and patients homes, across Coventry Out of Hospital setting, this service operates from Monday to Friday 8.30am to 5pm.

Would you like to be part of a team dedicated to improving the care and treatment outcomes across a wide range of patients whilst continuing to develop specialist skills in wound care?

We currently have vacancies within our Wound Healing Service. We are a specialist Wound Service focusing on Hard to Heal wounds, working with each patient on realistic goals depending on their wound healing potential.

We deliver advanced wound healing treatments within patients own home or residential/nursing home and within a clinic setting for ambulatory patients. We also work alongside Practice Nurses, Community Nurses and nursing homes offering lots of variety to the role with imparting wound care knowledge across the community. 

The role involves working within a variety of settings including clinic, community or residential home settings depending on the patient’s needs, delivering advanced wound healing interventions.
To work as an engaged member of the Wound Healing Service to provide effective evidence based Wound healing treatments and advice to patients who attend the Wound Healing Service.

Main duties of the job

 

§  Providing appropriately delegated care to patients receiving care within the designated service as directed by an RGN in line with the staff member’s competence.

§  Participate in the implementation, delivery & evaluation of nursing care plans to patients, their families and carers in community setting, including patients own homes.

§  Be responsible for reporting back to senior clinicians, on the care they have delivered, the outcomes for the patient and any other concerns they have about patient’s condition.

§  Adhere to Trust Policies, Procedures and best practice Clinical Guidelines.

Detailed job description and main responsibilities

 

Communication

 

·         Report to senior clinicians on the condition and needs of patients, families and carers, any delegated work not carried out, and escalate any concerns about the patient’s condition.

·         Participate in providing patients, relatives and carer’s education/information relating to service area for example, wound care & tissue viability and pressure ulcer prevention.

·         In instances of suspected safeguarding issues ensure that these are escalated to a senior clinician and reporting obligations of the HCA are fulfilled e.g., Incident Reporting

·         Ensure effective communication with all staff, patients, relatives, carers and system colleagues.

·         Ensure that all relevant information is relayed to clinical colleagues and senior clinicians at first opportunity including information on how individual’s needs are changing

·         Ensure that all documentation is completed as contemporaneously as possible and is stored in line with the Trust Record Keeping Policy (Including Principle Standards).

·         Maintain and enhance working relationships with other disciplines and promote effective communications within the Multi-Disciplinary Team.

·         To ensure & respect patient confidentiality in line with the Coventry and Warwickshire Partnership Trust Policy on Confidentiality.

·         Provide education/ information to patients’ relatives, carers within the remit of the of the non-registered clinician role.
