Clinical Sister/Charge Nurse - Post Renal Transplant clinic

Job overview
The Post Renal transplant clinic is looking for an experienced band 6, who has experience or exposure to the post transplantation care of Renal patients.
We monitor and look after patients who have had a kidney transplant for up to 6 months post. The successful post holder will be dynamic, flexible and have a positive attitude.
We work closely with are Multidisciplinary team to deliver the best holistic care for this group of patients. We are a small team of 7, team working, using your own initiative, thinking on the spot are skills required for this post.

Main duties of the job
- Knowledge of Post Transplant care/ Renal experience
- Good communication skills.
- A passion for providing high-quality care.
- Manage expected and unexpected clinical events requiring varying levels of physical effort according to patient dependency/ clinical need.
- Support patients, carers and others during difficult situations arising in the clinical area, e.g imparting bad news or following an unexpected event
- Promote and monitor adherence to Health and Safety and Trust policy designed to protect healthcare staff and service users from known hazards.
- Maintain a clean, safe environment ensuring adherence to trust standards of cleanliness, hygiene and infection control at all times.

Detailed job description and main responsibilities
1. Accountable for own actions in accordance with the Code of Professional Conduct. Will contribute to corporate objectives, acting within local, trust and statutory guidelines and policies at all times.
2. Effectively communicate information regarding clinical decisions, policy and care pathways to patients/ carers and the multi-professional team.
3. A skilled practitioner and clinical leader providing support to the management team, participating in service/policy development.
4. Responsible for clinical assessment of a group of patients and management of a team, ensuring appropriate care is planned, implemented and evaluated/audited.
5. Organisation of cross-departmental/organisational activities including HR, resourse, and bleep holder responsibilities as appropriate.
6. Demonstrate highly technical and/or practical skills to ensure optimum patient management including; complex patient monitoring, equipment management and frequent administration of invasive tests/procedures.
7. Develop and educate staff, participating in supervision, mentorship and appraisal as required.
8. Utilise and ensure others utilise IT systems to secure accurate and timely patient, workforce and resource data available.
