Patient Flow & Discharge Assistant

Job overview

An exciting opportunity has arisen to join the Emergency Medicine Group, primarily focused on our acute medicine wards as a patient flow and discharge assistant. 

This role will support the flow coordinators and clinical staff on the ward, in ensuring all patients have next steps in the care pathway, whilst liaising between different staff groups and services to support in timely, safe and effective way. 

Each day will vary and this is an excellent opportunity for someone with a passion for supporting patients and patient flow.

Main duties of the job

The post holder will act as a competent and integral part of the team, supporting the medical and nursing team in the flow of patients across the organisation from admission to discharge.

Key responsibilities will include visiting the wards and departments, joining board rounds and liaising with key members of the multidisciplinary team to gain intelligence an actual or potential discharges and or barriers to discharges, supporting in the resolution or escalation of these as appropriate.

The post holder will be able to utilise and update the various IT systems available in the Trust to support the smooth flow of patients across the organisation.

Detailed job description and main responsibilities

1. Support the operations team in meeting national targets for urgent and emergency care, such as 4hr and ambulance turnaround times.

2.  Be based on wards and visit other clinical areas frequently, communicating effectively with staff to maintain good relationships.

3.  Gain respect of and develop close working relationships with clinical and non-clinical staff.

4.  Be part of an MDT within the wards, ensuring patients always have a next step identified and escalate appropriately when not, helping to facilitate the safe and timely discharge of patients.

5.  Communicate and liaise with various departments, staff, and management teams.

6.  Able to provide timely and accurate information regarding bed availability and discharge plans.

7.  Able to communicate clearly both verbally and written.

8.  Liaise with the emergency medicine manager of the day, flow coordinator, medical and nursing teams to plan effective bed utilisation to create and maintain patient flow. 

9.  Maintain the electronic record systems in line with capabilities, to ensure that accurate information and bed requests is maintained.

10. Assist with moving and transferring patients in line with the Trust’s bed management policy.

11. Assist in the management of patient flow through working collaboratively across ED, wards, and departments and assist with the provision of effective bed utilisation.

12. To support the Manager of the day on duty by escalating delays in patient discharges.

13. Liaise directly with the Patient Transport Services to ensure early collection of patients ready to be discharged.

14. Ensure that confidentiality is maintained at all times in line with Trust policy.

15. Recognise limitations in clinical skill and operational knowledge and undertake tasks or make decisions which are within the remit of the role.
