Community Children's Nurse Band 6 - 37.5 Hours Per Week.

Our children’s community nursing team is part of the wider children and young people’s community nursing service. You will join a team who are passionate about delivering excellent clinical nursing care to our patients in their home, in our special schools, and our residential respite setting. The role offers a valuable opportunity to be part of an innovative service, to display your nursing expertise and make a difference where it matters.

We are looking for compassionate and committed colleagues. You will be able to demonstrate experience in nursing children and young people with a variety of medical conditions, including those with complex and palliative physical health needs. You will have an extensive portfolio of clinical nursing skills which will support the needs of our patients. If you share our passion for excellence, then we would love to speak to you.

Prif ddyletswyddau'r swydd

You will work autonomously managing a caseload of children and young people assessing, planning, implementing and evaluating their nursing care. Your activities will be varied, including, but not limited to delivering clinical interventions, training parents, carers and staff from other agencies,  and working with multi professional and multi-agency colleagues to support our children to achieve their best outcomes.

Our wider service is extensive and varied and can offer opportunities to work with children who are technology dependant, children’s palliative care, respite care for medically complex children, children’s continuing care and medically complex children in our special school nursing. No two days are the same, but each day counts.

You will work in collaboration with your colleagues to deliver end of life care to children at home, when this is their chosen place of death. To do this you will be part of our 24 hour on call rota and will be supported by a compassionate and caring team who care for each other as we care for our patients.
