Job overview

Leading and managing a pharmacy team to achieve highly effective pharmaceutical services, while simultaneously delivering patient centred care to the Cardiology and Cardiac Surgery departments across UHCW NHS Trust. Provide safe, cost-effective, specialist clinical pharmacy services to allocated wards and decentralised dispensing services, through effectively engaging with all members of the multidisciplinary team and seeking to improve the patient experience.

Main duties of the job

1. Provide effective clinical and medicines optimisation service to patients admitted and discharged through the Cardiology and Cardiac Surgery (cardiothoracic) department of the Trust.  This includes leading on prescribing, administration and governance within the cardiac theatres and cardiac catheter lab.

2. Liaise with Pharmacy Managers/Team Leaders, Senior Clinicians and Senior Nurse Managers in Cardiac services, to co-ordinate and ensure all services from pharmacy provide the best in seamless pharmaceutical care for these patients, innovating service change when necessary to achieve this.

3. Ensuring that wards and duties are clinically and operationally covered, while considering the needs of the service, and the requirements of the team members allocated to Cardiology and Cardiac Surgery department.

4. Provide information on trends in drug expenditure in relation to the drug budget.  This also includes horizon scanning and supporting new drug evaluations through the Trust Drugs and Therapeutics Committee.

5. Ensure best practice and clinical governance is applied to drug therapy by providing medical and nursing staff with:
•    Appropriate evaluated drug information
•    Support in education and training
•    Identify where gaps exist, support in writing and the implementation of clinical guidelines
•    Guidance on clinical audit and practice research.

Detailed job description and main responsibilities

1. Check prescriptions for clarity, safety and efficacy on wards, in dispensaries and the aseptic laboratory and when necessary take accurate drug histories to facilitate this.  Query with, and provide the prescriber with appropriate information to effect a change in the prescription when clinically required, to ensure patients’ safety and effective therapy
2. Provide when necessary, through checking dispensing or supervision of technicians, an efficient medication supply service from any of the pharmacy departments, including the aseptic laboratory and trials/Homecare services.
3. Ensure patients discharge medication is clinically appropriate, accurate and when possible, dispensed in advance of the patients discharge. Reduce pharmaceutical waste by utilising patients own drugs in the discharge process.
4. Provide information to patients concerning their medication and answer any resulting questions or queries.
5. Work flexibly across all of the Pharmacy departments, covering the work of colleagues during peak periods or absence as requested by Clinical Leads or the Senior Management Team.

6. To participate in the competency based assessments and training of junior staff within the team. To ensure areas of concern in the clinical and operational performance of staff within the team are identified and reported to the Clinical Leads. To participate in performance management of members of the team as needed.

7. Participate in appropriate training and development activities including your own personal development and participate in the education and training of colleagues and all other healthcare professionals on medicine legislation, pharmacology, medication risk and best prescribing practice. 

8. Contribute to local undergraduate and postgraduate education and training within your speciality, both within the Trust and for other programmes of study. 

9. To work with the Lead Pharmacy Technician/Pharmacist for Education and Training to ensure the effective delivery, assessment and induction of staff in the department. 

10. Act as a Co-ordinator for Keele University Clinical Pharmacy Diploma and mentor for other pharmacists within the Trust.

11. Participate in own appraisals and personal reviews and work to achieve agreed set objectives.  Perform appraisals for other pharmacy staff.

12. Share responsibility for allocation of work for pharmacists and supervise on a day to day basis.  

13. Participate in recruitment of staff.

14. Identify and undertake clinical audit and research appropriate to specialist area.

15. Participate in weekend, Bank Holiday, on-call working as may be required.

16. Undertake any other duties as required by the Director of Pharmacy in accordance with the grade and nature of the post.

17. Support the Director of Pharmacy in achieving long-term objectives of the department and its strategic direction including devising and reviewing SOPs and guidelines as needed.

18. Understand that the Trust is aiming to deliver 7 day a week services and that changes in shift pattern may be required in the future.
