An exciting opportunity has arisen for a physiotherapist to join the MDT within Eden Court Enhanced Care Facility which is an NHS Rehabilitation Services based within Wheatfield Court.

We are 20 bed rehab unit in the community offering ‘step down’ from hospital following acute illness as a stepping stone to home and ‘step up’ from community as admission avoidance.

We work as MDT with daily board rounds to update and plan care delivery. The team is made up of: consultant geriatricians, general practitioners, nurse practitioners, liaison nurse, podiatrists, physiotherapists, occupational therapists, dietician, rehabilitation assistant, social worker and speech and language therapist.

Prif ddyletswyddau'r swydd

As part of the therapy team, your role will include:

  • Comprehensive assessment and treatment planning
  • Individualised rehab programmes working with the patient and the MDT towards appropriate goals.
  • Discharge planning
  • liaison with other teams for information gathering
  • Supporting cover across Connie Lewcock Resource Centre- a similar rehabilitation unit based in Lemington.
  • Participating in the MDT rota providing weekend cover to Eden Court.

As a Physiotherapist within Community services, that may be a requirement to support other teams as required.

  • HCPC registration
  • Current UK driving licence

Knowledge & Experience

  • Comprehensive post registration experience as a junior Physiotherapist in NHS
  • Community working and lone working experience
  • Working as part of an MDT
  • Advanced knowledge of a range of medical conditions and conditions associated with older people and their impact on functional abilities
  • Sound knowledge of physio process
  • Critical decision making
  • Knowledge and experience of applying rehabilitation/Reablement models of intervention
  • Working with carers; instruction, advice, support
  • Risk assessment and management experience
  • Commitment to client centred practice for older people
  • Delegation to Support Staff

  • Awareness of challenges associated with community working
  • Knowledge of best practice related to older people/intermediate care
  • Experience of musculoskeletal orthopaedics

Skills & Abilities
