Job overview

This is an excellent opportunity to work in Newcastle upon Tyne Hospitals NHS Foundation Trust.  This exciting role will help support individuals with, or at risk of, Liver Disease to access treatment. Liver Disease is the third leading cause of premature death in the UK and the North East has the highest proportion of hospital admissions in England. The Patient Coordinator will be passionate about improving health outcomes for patients and reducing health inequalities in the region.

Main duties of the job

The successful candidate will form part of an award-winning team at Newcastle Hospitals, working alongside our clinical and administration colleagues. As a member of the multidisciplinary team (MDT), the Patient Coordinator will support patients throughout their healthcare journey, from identification to treatment. The postholder will demonstrate a patient focused approach that ensures excellent record keeping and communication between patients and the wider team.

Detailed job description and main responsibilities

  • The purpose of this role is to support and encourage individuals living with, and at risk of, Liver Disease, but who are not yet engaged in treatment, access healthcare services.
  • Form strong professional relationships with other clinical teams and partner agencies and work closely with them to identify patients who may benefit from the Liver Cancer Surveillance pilot, and ensure that they access treatment.
  • They will support the patient through the journey from identification and referral to conclusion of their intervention, thus eliminating barriers to care.
  • The Patient Coordinator will work closely with the wider Hepatology MDT to ensure a patient focussed service and will keep accurate patient and activity records to facilitate reporting back to
    Commissioners regarding performance and service success.

Qualifications & Education

Essential criteria
  • Hold NVQ level 4 or equivalent qualification / experience
  • ECDL advanced or equivalent experience o Computer literate with a good working knowledge of Microsoft packages. o Experience of using spreadsheets to a high level
Desirable criteria
  • Mental Health First Aid Qualification

Knowledge & Experience

Essential criteria
  • Experience of working with substance/alcohol misuse/liver disease issues and a clear understanding of the need for and ability to deliver quality services
  • An understanding of liver disease and its associated causal factors
  • Possess a sound working knowledge of hospital-based information systems, data collection, validation, and reporting
  • Demonstrate a broad understanding of liver disease issues
Desirable criteria
  • Demonstrate knowledge of liver disease identification programmes i.e. aims / outcomes / targets and deadlines

Skills & Abilities

Essential criteria
  • Possess excellent communication skills and be able to work to deadlines
  • Possess a comprehensive range of practical analytical skills
Desirable criteria
  • Training in Brief Motivational Interventions
