Sylwer bod ychwanegiad dros dro ar gyfer Bandiau 1,2 a 3 i adlewyrchu ymgorffori'r ychwanegiad i’r cyflog byw, sef £12 yr awr - £23,465 y flwyddyn. Bydd yr ychwanegiad dros dro hwn yn weithredol hyd nes y bydd y codiad cyflog blynyddol ar gyfer 2024/25 wedi’i gadarnhau.


Gellir cyflwyno ceisiadau yn y Gymraeg. Ni fydd ceisiadau a gyflwynir yn y Gymraeg yn cael eu trin yn llai ffafriol na chais a gyflwynwyd yn y Saesneg.


 

Trosolwg o'r swydd

THIS POST IS FIXED TERM/SECONDMENT UNTIL MARCH 2025 DUE TO WELSH GOVERNMENT FUNDING.

This is an exciting opportunity for a passionate, motivated nurse to join the Primary Care Nursing team and liaise directly with a defined group of Primary Care services. The aim is to enhance the knowledge, skills and support of staff across our general practitioner services to ensure that they are able to provide safe and effective care to people with learning disabilities via the mandatory annual reviews.

Learning Disabilities directed enhanced services delivered in our general practitioner services across Swansea Bay encourage practice teams to adopt a more proactive approach. Spending more time with patients with learning disabilities and their carers enabling health problems to be established, and treated at the earliest possible stage, minimising risk to patients’ health. The post holder will be autonomous in facilitating the work closely with a range of Primary care health professionals, people with learning disabilities and their families to support the reduction of health inequalities experienced by this vulnerable group, promoting a team based approach to care.

Prif ddyletswyddau'r swydd

The main duties of this post include, providing knowledge and advice to colleagues within primary care to ensure the delivery of quality, evidence based learning disability health checks and improved patient outcomes. There will be further scope in the role to support training needs and working alongside the Primary care academy and wider LD community with any training or service gaps identified.

 The role requires the nurse to work in an autonomous way’ promoting best practice and providing advice, guidance and support across the GP footprint. Reporting on delivery of services in the Health Board, meeting the annual review requirement for the patient cohort on the GP register, and ensuring staff feel sufficiently trained to deliver this service.

Qualifications

  • Registered Nurse
  • Masters qualification or equivalent demonstrable specialist knowledge of mentorship, education and development practice.
  • Mentorship Module
  • Knowledge of primary care services and IT systems , with an understanding of the relevance and local impact of issues and policy from the wider arena of the nursing profession and the NHS.

  • Leadership Training
  • Teaching & Assessing/ PGCE qualification or working towards
  • Knowledge of Health inequalities in community services

Experience

  • Relevant clinical experience and demonstrate awareness of research-based practice.
  • Evidence of contributing to practice development and evidence of own professional development and relevant further study.
  • Motivate staff and maintain good working relationships and experience of staff development and performance management.
  • Experience in the development, delivery and evaluation of a programme/project
  • Prioritise own workload and use initiative.
  • Demonstrates understanding and engagement with quality issues, e.g. benchmarking.
  • Support staff during their PADR reviews and revalidation and support a conducive learning environment for staff and students.
  • Demonstrate a clear understanding of r a quality improvement and promoting patient outcome improvement initiatives.
  • Demonstrates an understanding of the scope of clinical governance issues.
  • Proven supervision/ mentorship skills.
