Please be advised that there is a temporary top up for Bands 1,2 and 3 to reflect the incorporation of the top up to the living wage of £12 per hour - £23,465 per annum. This temporary top up will be in place until the annual pay uplift for 2024/25 is confirmed.

Job overview
The successful applicant will be based in the Home Treatment Team.
The post holder will have an understanding of the knowledge and skills of the professional activities within a Mental
Health team in the unscheduled care service. They will be responsible for liaising with both Social & Healthcare
professionals to meet client need.
Contribute to the development, maintenance and evaluation of systems and structures, which promote the rights,
responsibilities and diversities of people with a mental health condition, through effective communication.
The post holder will promote the health and wellbeing of clients by providing specialist nursing assessment and advice
and training to clients, carers and other professionals.

Essential criteria
- RMN Current NMC Registration
- Evidence of Post Registration Education to Degree level or working towards.
Experience
Essential criteria
- Evidence of experience working within a multi-disciplinary/ multi agency context.
- Experience/ knowledge of risk management.
- Experience of working with families.
- Experience of working with individuals with complex health needs and those with challenging behaviour.

- Able to demonstrate ability to assess clients, develop and monitor care plans for a group of complex individuals.
