Mae Bwrdd Iechyd Prifysgol Bae Abertawe yn cadw’r hawl i gau’r swydd wag hon ar ôl 24 os na dderbynnir nifer fawr o geisiadau addas. Rydym felly’n hybu ceisiadau cynnar i sicrhau ystyriaeth ar y swydd hon.

Bydd staff sy’n bresennol yn aros am adleoliad yn cael eu hystyried yn gyntaf ac felly rydym yn cadw’r hawl i dynnu’r hysbyseb hon yn ôl ar unrhyw adeg.

Sylwer bod ychwanegiad dros dro ar gyfer Bandiau 1,2 a 3 i adlewyrchu ymgorffori'r ychwanegiad i’r cyflog byw, sef £12 yr awr - £23,465 y flwyddyn. Bydd yr ychwanegiad dros dro hwn yn weithredol hyd nes y bydd y codiad cyflog blynyddol ar gyfer 2024/25 wedi’i gadarnhau.


Gellir cyflwyno ceisiadau yn y Gymraeg. Ni fydd ceisiadau a gyflwynir yn y Gymraeg yn cael eu trin yn llai ffafriol na chais a gyflwynwyd yn y Saesneg.


 

Trosolwg o'r swydd

The post holder will have responsibility to provide a single point of access for all referral into Children's and Adolescent Mental Health services.

To continue to review all referrals to identify any gaps in the referral process with a view to strengthening existing joint partnership systems to improve early and appropriate referrals.

To collate the statistical data from the current database to facilitate the process.

To triage referral information, making autonomous and joint clinical judgements in order to prioritise and allocate the referral to the appropriate pathway within our service or suggest alternatives to the referrer. This is completed in line with the Mental Health Measure (Wales) 2010.

Triaging phone calls from external agencies and taking duty calls from the general public at times. Provide brief interventions during the telephone calls.

Conducting holistic assessments. Formulating risk assessments.

An exciting opportunity has arisen within Child and Adolescent  Mental Health Services. The opportunity involves being part of a developing and expanding team to improve the access to mental health services, and facilitate swift and effective referrals to relevant services as needed.

The post is for a dedicated Single Point of Access Professional. The job is open to qualified mental health  professionals

The post will require the Professional to Triage all referral information, prioritise and allocate to the appropriate treatment pathway. The role will sometimes also include face to face screening.

The Single Point of Access Team is a developing team within the health board.  The team will encourage development and support.  The ethics of the team is to deliver high quality care and support to individuals experiencing mental health concerns. 

Varied shift pattern offered.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications

Meini prawf hanfodol
  • RMN registration, occupation therapy/social work
  • Possession of post graduate qualification (diploma, degree or higher qualification) or evidence of working towards the same.

Experience

  • Substantial post qualification experience as and Band5/6 or equivalent working within the field of acute adult/ community mental health.
Meini prawf dymunol
  • Experience of teaching and training other health and / or social care professionals and designing educational inputs (this could include conference presentations, publications etc).
