Mae Bwrdd Iechyd Prifysgol Bae Abertawe yn cadw’r hawl i gau’r swydd wag hon ar ôl 24 os na dderbynnir nifer fawr o geisiadau addas. Rydym felly’n hybu ceisiadau cynnar i sicrhau ystyriaeth ar y swydd hon.

Bydd staff sy’n bresennol yn aros am adleoliad yn cael eu hystyried yn gyntaf ac felly rydym yn cadw’r hawl i dynnu’r hysbyseb hon yn ôl ar unrhyw adeg.

Sylwer bod ychwanegiad dros dro ar gyfer Bandiau 1,2 a 3 i adlewyrchu ymgorffori'r ychwanegiad i’r cyflog byw, sef £12 yr awr - £23,465 y flwyddyn. Bydd yr ychwanegiad dros dro hwn yn weithredol hyd nes y bydd y codiad cyflog blynyddol ar gyfer 2024/25 wedi’i gadarnhau.


Gellir cyflwyno ceisiadau yn y Gymraeg. Ni fydd ceisiadau a gyflwynir yn y Gymraeg yn cael eu trin yn llai ffafriol na chais a gyflwynwyd yn y Saesneg.


 

Trosolwg o'r swydd

An exciting opportunity as arisen within the Long Term Care Team in Primary Care & Therapies Service Group. The successful candidate will be responsible for managing the effective implementation and governance of the Health Board’s policy on long term care, including Continuing Health Care and Funded Nursing Care. 

Prif ddyletswyddau'r swydd

The Long Term Care Manager is accountable within the appointing area, however, will be required to work in partnership across the Health Board to develop consistent approaches and make best use of the skills and expertise that are available in the best interests of the organisation. The individual will manage a diverse team of staff and have responsibility for a significant delegated budget.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Swydd ddisgrifiad a phrif gyfrifoldebau manwl

·       The post holder will be responsible to the Head of Long Term Care, for the effective delivery of a number of long term care services that requires the interpretation of local and national policies, advising the Health Board on their implementation, in order to achieve corporate objectives, and will take a key role in designing and developing new models of long term care.

Qualifications

  • registered nurse
  • degree level or equivalent
  • Master's degree or working towards
  • postgraduate management qualification or professional qualification or equivalent experience

  • Management qualification or equivalent experience as degree course with management component
  • ECDL qualification
  • ability to speak Welsh

Knowledge

  • currently working within long term care services
  • in-depth knowledge of FNC & CHC framework and guidance
  • up to date knowledge of challenges facing NHS and policy context within Wales

Experience

  • management role for minimum 3 years
  • Experience of working effectively with partners, stakeholders and the public to deliver change.
  • Experience of financial planning and the ability to manage budgets effectively to meet financial targets.
