NHS Frimley Integrated Care Board was formed on 1 July 2022 under the Health and Care Act 2022, replacing NHS Frimley Clinical Commissioning Group.

Job overview
An exciting opportunity has arisen for someone to be part of the Medicines Optimisation in Social Care and Care Homes Team within Frimley ICS. This team is well established and highly respected locally as well as nationally. The team leads on ensuring that medicines use in care homes and other social care settings is individualised, safe, effective and sustainable.
The role is for a Pharmacist to work at an advanced clinical level, providing advice and support to care homes and social care organisations across the system, developing best practice guidance, delivering training to all levels of staff and carrying out complex structured medication reviews and supporting and developing PCN and GP practice pharmacy staff to carry out to enhance outcomes for people in care homes and social care settings.
Main duties of the job
This role is part of the Medicines Optimisation in Social Care and Care Homes Team within Frimley ICS. This team is well established and highly respected locally as well as nationally. This job description is only an outline of the tasks, responsibilities and outcomes required of the role. The role will evolve as needed to support the emerging pharmacy and medicines optimisation transformation agenda and to the needs of the local system. As such, the postholder will be required to carry out any other duties appropriate to the role.

Person specification
Application and Interview
Essential criteria
- Registered with the GPhC as a pharmacist
- Active engagement with relevant continuing professional development
- Educated to Post-graduate Diploma or Master’s level; or demonstrable equivalent level of experience working at a senior level
- Post-graduate qualification in clinical or advanced pharmacy practice or equivalent; or demonstrable experience of working in a clinical role
- Qualified as an Independent Prescriber or planning to undertake the qualification in the near future
- Line management training or experience in line management
- Aware of competencies set out in Advanced Pharmacy Framework and working towards them
Applicant requirements
You must have appropriate UK professional registration.
The postholder will have regular contact with vulnerable people and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
