NHS Frimley Integrated Care Board was formed on 1 July 2022 under the Health and Care Act 2022, replacing NHS Frimley Clinical Commissioning Group.

The ICB is the new statutory NHS organisation responsible for planning and delivering health and care services. It will work collaboratively with partner organisations including the voluntary, community and social enterprise sector, people and communities across the Frimley Health and Care Integrated Care System (ICS).

NHS Continuing Healthcare (CHC) is a package of continuing care that has been arranged and funded solely by the NHS to meet a person’s physical and mental health needs arising from disability, accident or illness.  It is for vulnerable people with complex, intense and unpredictable needs or people who are rapidly deteriorating and possibly entering a terminal phase.

NHS Frimley ICB’s East Berkshire Continuing Healthcare Service is seeking to recruit a highly motivated, very well organised and experienced registered nurse with qualifications in Learning Disabilities to join the team of CHC assessors to support in the processing of Continuing Healthcare and Funded Nursing Care assessments, reviews and case management.  You will have knowledge of the National Framework for Continuing Healthcare (Oct 2018) and be able to:

 

•            Coordinate multi-disciplinary meetings with a range of professionals i.e. social care, care homes,             other providers etc.

•            Undertake high quality holistic assessments of patients, clearly identifying needs using assessment         tools as required.

•            Manage a caseload of complex cases.

•            Cover as Duty Nurse within the Continuing Healthcare Service.

 

You will be a key member of the team and will need to demonstrate skills in IT, report writing, managing complex patients. You will also have excellent verbal and written communication and have the ability to work in stressful situations and be able to prioritise workloads to meet deadlines.  You will be working in partnership with our local authorities and experience of multi-disciplinary working is essential. 

 

A current driving licence and vehicle used for business use will be required.

Knowledge, Training and Experience

  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
  • First level qualification RNLD Registration
  • Further training or significant experience in project management, financial management or supporting change management processes
  • Experience and understanding of evaluating and measuring the performance of health services.

  • Palliative care experience

  • Experience of working in older persons care in NHS or Independent setting
  • Experience of assessing complex needs and planning and evaluating complex care packages
  • Knowledge and understanding of safeguarding of vulnerable adults
  • Audit data analysis- ability to analyse data and use it for audit purposes.
