South West London Neuronavigator; hosted by Sutton Community Neuro Therapy service
We have an exciting opportunity to join the pilot ‘SW London Neuronavigation Service’, as a Neuronaviagtor hosted by the Sutton Community Neuro therapy service (Host Sutton Health & Care, Epsom & St Helier NHS Trust).
You will be joining an experienced clinician in a job share capacity working 3 days per week (Mon, Weds, Thurs).
The SW London Neuronavigation service commenced in May 2022 & consists of four neuronavigation posts, hosted within the community neuro teams across SW London. The postholder will be affiliated and work closely with named teams, there is an expectation that they will work across teams as well as supporting/represent all six SW London Boroughs as required.
This role is primarily aimed at improving the experience of people with highly complex long term neurological disabilities, as they journey though the health and community care service system.
The key aspects of the role are defined as ‘providing proactive case management and direct support to the local acute and rehabilitation services, with expert clinical advice, assessment and education, and directing patient referrals to the most appropriate specialist neurorehabilitation service with the emphasis on enhancing flow’ (London Neuronavigator Service Operating Model).

Applicants will be considered from any related discipline including Occupational Therapy, Physiotherapy, Speech and Language Therapy, Psychology, Nursing or Social Work.
The successful applicant will have extensive experience working in a range of specialist neurology/neurorehabilitation settings. This is a pilot project, confidence and competence in using excel and experience in audit and data collection is also important.
All of the Community Neuro Teams provide multi-disciplinary assessment, treatment, advice and information to adults with a variety of progressive and non-progressive neurological conditions. The services also offers early supported discharge for stroke patients and link closely to local acute hospitals.
The team compositions vary slightly but most have Occupational Therapists, Physiotherapists, Speech and Language Therapists, Clinical Neuropsychologist, Dietitian, Clinical Nurse Specialist, Neuronavigator/Neuro Case Manager, Psychology Assistant, Therapy Technical Instructors and Admin Assistants.
As a senior clinician the post holder will have a lead role in the development of the Neuronavigation role/service within the multi-disciplinary teams and across SW London, and will also have a key role in wider neuro rehab service development.
Continuing professional development is actively encouraged and is supported via clinical supervision, annual PDP Plans, post-graduate and in-service training . All our staff are fully supported by a Clinical Team Lead.
