Job overview

This is an exciting opportunity to work with the Sutton Virtual ward in a new role which combines clinical duties, both in St Helier Hospital and community, with line managing a team of Urgent Response Nurses. 

Working closely with the multi-disciplinary teams you will assess patients in their own homes who have been discharged early from hospital or to avoid admission where possible.  You will also work within St Helier hospital, identifying and assessing suitable patients, building relationships and educating colleagues. 

You will be instrumental in building trust and confidence in the Virtual Ward offer and developing new and existing clinical pathways.

 

Main duties of the job

  • Undertake advanced holistic assessment of patients in the community or hospital environments
  • Line management responsibility for a group of Band 6 nurses (sickness, performance management, training and development)
  • Assisting the operational manager with rota planning, annual leave etc. 
  • Lead on audits and reporting as required by the operational manager
  • Take an active part in MDT meetings and decision making for patients under the care of the Virtual Ward
  • Work closely with the Virtual Ward team to ensure smooth day-to-day running of the Virtual Ward
  • Take responsibility for own professional development 
  • Ensure a balance of clinical and non-clinical tasks, prioritising and managing own time.
  • Act as an enthusiastic and knowledgeable representative of the Virtual Ward

Education/Qualifications

Essential criteria
  • Specialist Practitioner qualification/Advanced Practitioner at degree/masters or equivalent
  • Evidence of continued professional development
  • Prescribing as part of specialist practitioner degree or V150/V300
  • Physical Assessment and Clinical Reasoning skills
Desirable criteria
  • Specialist practice teacher qualification

Experience

Essential criteria
  • Recent post registration experience as a senior nurse in the community setting
  • Experience in managing the needs of complex patients in a community setting
  • Experience in supervising, mentoring and appraising staff
  • Assessment and Supervision of pre-Reg students
  • Experience of working autonomously

Skills and Abilities
