Ambulatory Emergency Care Services have been available on the Wexham Park site for 10 years and are robust, successful and vital aspects of the urgent and emergency pathway on both sites. As primarily admission avoidance services, there function and processes have been part of many national publications and pilots and these services at FHFT are held amongst the top performing services of their kind in the NHS.
In November 2022, we launched our Urgent & Emergency Care (UEC) Virtual Ward to offer an expansion to the options available within FHFT for Acute Medical same day emergency care (SDEC), providing admission avoidance and early discharge for those patients managed through these services that could be managed virtually, increasing both inpatient and SDEC capacity to support flow in the emergency pathway.
Since launching, we have managed over 4000 patients through our service and have proven the benefits it brings to the Trust and patients. With ongoing pressures facing our Urgent & Emergency pathways, we have successfully been given funding to expand our service and so we are looking for talented and enthusiastic individuals to join our growing team.

Main duties of the job
These posts will be based at the front doors of our Acute hospitals and key to help support and champion the use of virtual care and maximise the ability to give a further alternative to admission by streamlining referral processes and provide more capacity for earlier assessment and discharge.
The other primary function of these roles will be to actively recruit and monitor patients on the UEC Virtual Ward caseload alongside our growing team of Advanced Clinical Practitioners and in conjunction with our Acute Medical and SDEC teams.

Individuals will have experience in acute care areas and hold specialist experience and education in the assessment and management of patients as per the attached job description.

Qualifications
Essential criteria
- Current unrestricted registration as a Nurse, Midwife or AHP
- Completed advanced history taking/patient assessment module at level 7 or plans to complete in next 6 months.
- Mentorship/teaching course or willingness to complete in first 6 months
- GCSE English and Mathematics at grade C or above (or equivalent qualifications)
Desirable criteria
- Leadership qualification/course
- Resuscitation/Life Support course (if appropriate to area and role)
- Non-medical prescribing qualification (relevant to role and discipline)
Experience
Essential criteria
- Experience within the speciality at a band 6 or equivalent
- Evidence of ability to work at a more advanced practice level.
- Management of patients with complex needs
- Evidence of delivery of training
Desirable criteria
- Management & Leadership experience and band 6 or 7 level
- Experience of teaching to large groups
- Contribution to audit
Skills & Knowledge
Essential criteria
- Evidence of continuing professional development
- Knowledge of current practice issues and interest in further learning
- Evidence of good written and oral communication skills including presentations
- Able to recognise and analyse complex situations and take appropriate solutions
- Good IT skills with ability to present information appropriately
Desirable criteria
- Ability and initiative to develop specialist areas of interest
- Awareness of critical analysis & awareness of application of research to practice
