Community Rehabilitation Practitioner for Acute Frailty

Full time

We are looking for an enthusiastic and hard working person to work across the Acute Frailty Service in the Hospital at Home and Urgent Community Response within the Royal Surrey NHS Foundation Trust, based in Milford Hospital.

Main duties of the job

You will work as part of a multidisciplinary team alongside therapists, doctors, Advanced Care Practitioners, nurses and pharmacists to provide integrated and personalised care to the patients.

We aim to provide hospital based care in the home environment avoiding unnecessary admissions to the acute hospital and supporting early discharges home.

We are a relatively new service with lots of opportunity for service and personal development.

You will receive an annual appraisal and personal development plan, enabling you to have a structure within your role and help you develop personally and professionally.

Must hold a valid UK driving licence and have use of a car.

Detailed job description and main responsibilities

Working as part of the Hospital at Home team which provides short term support to people being discharged from hospital or to keep them in their own home, the team work with our patients to enable them to achieve their optimum independence.

Patients will require support with personal care, mobility, medication administration and exercise programs to achieve their individual recovery and rehabilitation goals .You will take on new patients on the Hospital at Home caseload and help with the planning of their care, supported by our nurses and therapists if required for complex clinical decision making.

The post holder will be key to supporting the wider team and ensuring visits are all completed on the day, helping with allocation, scheduling and adjustments throughout the day.

Essential criteria
  • NVQ 3 level 3 equivalent of knowledge and experience
  • Portfolio evidence of relevant experience/ CPD in specialist field

Knowledge and Skills

Essential criteria
  • Able to work on own initiative and recognise when to seek assistance
  • Broad experience of working as a Senior Community rehabilitation Assistant with some experience supporting more junior staff ( band 2) or relevant Carer experience
  • NHS experience within acute &/or community setting
  • Recent experience of working as part of a multidisciplinary team, with an understanding of roles and responsibilities of the team.
  • Understanding of common medical conditions and terminology
  • Able to perform a basic primary assessment according to unit guidelines
